Zevra Therapeutics (ZVRA) Q3 2026 earnings review
Zevra's US drug sales rise, but overseas access revenue drops
Zevra Therapeutics, a maker of rare-disease drugs, grew its US flagship drug while overseas early access revenue tumbled. Total net revenue rose 45% from a year ago to $38 million, or 52% by our math excluding a terminated royalty stream, as chief executive Neil F. McFarlane pointed to continued US prescription gains. However, total sales slipped 5% from last quarter because overseas early access reimbursements dropped from $9 million to $4 million.
| Sales from ongoing businesses | +52% from a year ago (reported revenue $37.8M, +45%) |
|---|---|
| US MIPLYFFA net revenue | $34.0M +52% from a year ago; +13% from last quarter |
| Global Early Access net revenue | $3.6M -60% from $9.0M last quarter |
| Financial resources outlook | Unchanged $270.9M in cash and investments, up $10.7M |
⚖️ Verdict: 🔴 Bearish
The story got worse because the drop in overseas access sales confirmed that the past two quarters relied on uneven bulk orders rather than a steady second growth engine. On the other side, US MIPLYFFA sales rose 13% from last quarter to $34 million on 13 new prescription forms, and Zevra plans a 2027 filing to treat infants.
The question now is whether European regulators reverse their initial rejection of arimoclomol and whether the FDA allows a faster path for the celiprolol heart-vessel trial. Positive decisions would open commercial European sales and shorten a six-year trial wait, while refusals would leave Zevra reliant on US MIPLYFFA and lumpy overseas orders. The fourth-quarter European re-examination ruling and conference call details on the recent FDA meeting will settle both points.
🐂 Bull Case
US MIPLYFFA Sales Reached $34 Million
MIPLYFFA is Zevra's approved medicine for Niemann-Pick disease type C (NPC), a rare and fatal brain disorder. Chief executive Neil F. McFarlane said the rare-disease drugmaker is "building on MIPLYFFA's commercial performance in the U.S. and advancing our pipeline in support of our mission to redefine what’s possible". US net revenue from the drug reached $34.0 million this quarter, up 52% from a year ago and 13% from last quarter.
Doctors submitted 13 new prescription enrollment forms during the third quarter, lifting the total since launch to 197. By our math, US sales per cumulative form rose to about $173,000, the highest rate since launch. Following a meeting with the US Food and Drug Administration (FDA) "to discuss a potential supplemental New Drug Application (sNDA) for MIPLYFFA in infants less than 24 months of age", Zevra plans to file that application in the first quarter of 2027.
What to watch: fourth-quarter US prescription enrollment additions and active paid patient counts on the conference call. Adding a dozen or more forms while keeping revenue per form near $170,000 would show that US demand is not slowing as total forms approach the 300 to 350 diagnosed population.
Lower Overhead Share Lifted Margin and Cash
Zevra paid off its term loan in the first quarter and runs a small commercial team for rare diseases, so rising US drug sales flow directly into profit and cash. Operating income nearly tripled from a year ago to $11.6 million, lifting the operating margin by 14.8 percentage points to 30.7% by our math.
Selling, general and administrative expense — sales and overhead costs — grew 13% to $19.1 million on "commercial and infrastructure related costs", the company said. Because revenue grew much faster, sales and overhead dropped from 65.0% of revenue a year ago to 50.7% by our math. Total cash and investments rose by $10.7 million from last quarter to $270.9 million, with zero debt on the balance sheet.
What to watch: operating cash flow and capital deployment details in the quarterly filing. Continued cash generation above $10 million a quarter without share buybacks will keep building a large cash balance that management has yet to earmark for specific deals.
🐻 Bear Case
Early Access Revenue Dropped 60% Sequentially
"Our strong financial position enables us to advance these priorities while continuing to make arimoclomol available to eligible patients through our global Expanded Access Program," chief executive Neil F. McFarlane said. The Expanded Access Program (EAP) supplies arimoclomol — the active ingredient in MIPLYFFA — to NPC patients outside the US in France and other countries.
- Global EAP net revenue: $3.6 million, down 60% from $9.0 million last quarter by our math
- Enrolled EAP patients: 134, up by 2 from 132 last quarter
- Quarterly EAP revenue per patient: about $27,000, down from $68,000 last quarter by our math
This sharp drop shows that the $10.2 million and $9.0 million recorded in the first two quarters came from bulk supply orders rather than a steady quarterly run rate. Because non-US countries order on uneven schedules, the program dragged total company revenue down 4.8% from last quarter despite record US sales.
What to watch: fourth-quarter EAP net reimbursements and any split between French and other international orders. Another reading near $3.6 million would confirm that the recurring overseas base has settled back near the French program floor.
Celiprolol Trial Added Only One Clinical Event
Celiprolol is Zevra's experimental medicine for Vascular Ehlers-Danlos Syndrome (VEDS), a rare genetic disorder that weakens blood vessels. The drug is in an event-driven Phase 3 study called DiSCOVER, which requires 28 confirmed medical events among patients to trigger an interim review. During the third quarter, the company enrolled six patients to reach 72 in total and recorded one additional confirmed event, bringing the total to four.
At one confirmed event per quarter, reaching the 28-event mark would take roughly six more years, so Zevra wants a faster regulatory path. The rare-disease drugmaker recently met with the FDA "for a Type C meeting to discuss a potential submission strategy for celiprolol", but the release gave no outcome or timeline change from that discussion.
What to watch: management's readout of the FDA Type C meeting on the conference call. Without an agreement from regulators to lower the 28-event requirement, celiprolol will continue to consume research spending with no visible approval date.
Risks this quarter didn't answer
Four standing concerns around European approval, revenue concentration, patient conversion and overseas pricing got no new figures in the earnings release.
- European drug review: Zevra requested a re-examination of its European application for arimoclomol and "is awaiting the outcome of the process"; a fourth-quarter decision will determine whether it can sell the drug commercially in Europe.
- Single-drug reliance: arimoclomol generated 99.6% of total revenue this quarter by our math, leaving the business exposed to any single-distributor inventory shift.
- French pricing clawback reserve: the quarterly filing will show how much Zevra set aside for retroactive French price rebates on EAP sales.
- US patient-finding and retention rates: the release gave no conversion rates for genetic testing programs and no count of patients who stopped taking MIPLYFFA.
👓 Other Themes
Terminated Royalties and Warrant Swings Skewed Comparisons
Two accounting items distort the headline comparisons this quarter. Zevra ended its AZSTARYS royalty agreement in March 2026 after selling those rights, which removed $1.2 million of royalty revenue from the year-ago base; excluding that terminated stream, revenue from ongoing businesses grew 52% from a year ago by our math, compared with 45% reported. Net income of $19.2 million also included $5.1 million of non-cash income from warrant and contingent value right liability adjustments, leaving $14.1 million of profit before fair-value swings by our math.
💲 Other KPIs
Uncollected customer bills fell 11.3% from last quarter and rose 18.4% from a year ago by our math, growing much slower than the 44.9% rise in revenue. Lower receivables alongside record US drug sales indicate timely cash collection from the specialty pharmacy and earlier international access shipments.
The diluted share count rose 10.3% from a year ago and 0.6% from last quarter by our math. The year-over-year increase reflects warrant exercises earlier in the year, employee stock awards, and the inclusion of 2.3 million dilutive options and warrants now that the company is profitable.
Research spending rose 43.9% from a year ago and 10.1% from last quarter, holding steady at 13.1% of revenue by our math. Zevra spent more to fund ongoing arimoclomol studies and the Phase 3 trial for celiprolol.
🔮 Guidance
Unchanged. Zevra reiterated that "available financial resources are sufficient to execute on its strategic priorities" under its current operating plan. The drugmaker gives no numerical revenue or profit outlook, but its cash and investments grew by $10.7 million this quarter to $270.9 million as operating margin reached 30.7%.
❓ Key Questions
What did the FDA decide on the celiprolol trial?
Zevra held a Type C meeting with the FDA to discuss a submission strategy for celiprolol, but gave no details. With only 4 of 28 required clinical events confirmed, investors need to know if regulators agreed to a faster path.
How much of the $3.6 million EAP revenue is recurring?
Overseas early access reimbursements dropped from $9.0 million last quarter to $3.6 million, with just two new patients added. Management has not separated the stable French program from lumpy bulk orders in other countries.
How many US patients are actively taking paid MIPLYFFA?
Cumulative US prescription enrollment forms reached 197, two years after the September 2024 approval. Zevra still does not report how many forms converted into active paid patients or how many patients stopped treatment.
What is the French clawback reserve on the balance sheet?
Zevra books French early access revenue net of a price rebate reserve that totaled $18.6 million in June. Because the press release omits footnote details, the quarterly filing is needed to see how a pending European review affects that liability.
