Travere Therapeutics (TVTX) Q3 2026 earnings review
Travere's kidney drug sales jump, but combined numbers hide details
Travere Therapeutics, a rare kidney disease drugmaker, proved its spring launch surge was not a one-quarter spike as prescriptions held steady. US sales of its main pill Filspari jumped 42% from last quarter to $200 million, while doctors sent in 1,932 new patient start forms. A $113 million licensing fee pushed the company into a reported loss, but chief executive Eric Dube said the drug is still early in reaching over 100,000 eligible patients.
| US product sales | $220.2M +94.6% from a year ago |
|---|---|
| Filspari US net sales | $200.2M +41.9% from last quarter |
| Filspari new patient start forms | 1,932 2,012 last quarter |
| Operating profit, excluding one-offs and upfront fee | $73.7M by our math; -$68.1M reported |
⚖️ Verdict: 🟢 Bullish
The story got better because third-quarter prescription forms stayed near 2,000, easing fears that the second-quarter surge relied on a waiting list of pre-identified patients. Excluding the one-time licensing fee, operating profit excluding one-offs reached $74 million by our math and generated $95 million in cash before the deal payment. One caution: management still combines both kidney diseases into one prescription number and gave no update on price discounts.
The question now is whether older IgAN prescriptions are holding up or quietly losing ground to rival drugs behind the FSGS launch wave. Either both conditions are growing together, or rapid FSGS adoption is hiding doctors switching IgAN patients to competing therapies. Fourth-quarter prescription forms and any disease split on the call will show if total demand can climb past 2,000 starts.
🐂 Bull Case
Filspari Sales Topped $200 Million as Prescriptions Held Up
Travere sells Filspari, a daily pill for two rare kidney diseases: IgA nephropathy (IgAN) and focal segmental glomerulosclerosis (FSGS), a condition that scars the kidney's filters. After US regulators approved the drug for FSGS in April, quarterly new patient start forms—the paperwork doctors submit to begin treatment—doubled to 2,012 in the second quarter. Critics worried that first wave came mostly from waiting lists and would drop once doctors worked through those early patients.
Instead, demand held near that record in the third quarter even during the slower summer months. Doctors sent in 1,932 start forms, down 4.0% from the second quarter, while second-quarter enrollments turned into paid refills and lifted Filspari net sales 41.9% from last quarter to $200.2 million.
- Filspari US net sales: $200.2 million, up 120.2% from a year ago and up $59.1 million from last quarter
- Sales added per prior-quarter start form: $29.4 thousand by our math, above the $17–26 thousand range before FSGS
- Total US product sales: $220.2 million, up 94.6% from a year ago while total revenue rose 36.2% against a $51.7 million prior-year partner payment
The higher dollar return per form matters because FSGS patients move up to double the adult IgAN dose, which raises the price per patient. Because third-quarter start forms stayed near 2,000, the company enters the fourth quarter with a similar wave of new patients moving onto paid therapy.
What to watch: fourth-quarter Filspari net sales and start forms in February. Another sequential sales gain near $50 million would show that higher FSGS dosing is outweighing wider Medicare price discounts.
A Licensing Charge Hid Surging Operating Profit and Cash
On paper, the biopharmaceutical company swung from a profit a year ago to a $68.1 million operating loss, or a $39.5 million loss excluding stock-based pay and amortization. That drop happened because Travere closed its licensing deal with Everest Medicines for civorebrutinib, an experimental kidney pill, and booked the $113.1 million upfront payment as an immediate research charge equal to 50.4% of revenue.
Without that one-time licensing fee, operating profit jumped as drug sales outpaced marketing and research spending. By our math, operating profit excluding the upfront charge reached $45.0 million on a reported basis and $73.7 million excluding one-offs, up from $30.0 million in the second quarter.
- Selling and overhead costs: 44.7% of product sales, down 18.1 percentage points from a year ago by our math
- Research and development costs: 31.3% of total revenue, down 0.2 percentage points from a year ago by our math
- Cash generated before the $112.5 million Everest cash payment: $95.1 million in the quarter by our math
What to watch: fourth-quarter operating profit in February, when no upfront licensing charge hits the income statement. A clean quarterly profit above $50 million would confirm the commercial business can fund the clinical pipeline on its own.
New Patents Extend Filspari Protection Into 2037
Investors have questioned how long Filspari can fend off generic copies once its initial seven-year rare-disease exclusivity periods expire in 2030 and 2031, and whether Travere can build a second drug behind it. In August, the US Patent and Trademark Office granted a patent covering ways of using the drug in IgAN through October 2037, and in September it allowed a matching patent for FSGS into October 2037.
Alongside longer patent life for its main drug, the drugmaker moved three clinical programs toward key milestones. Management continues to enroll patients in the Phase 3 HARMONY study of pegtibatinase, an enzyme therapy it says could become the "first and only disease-modifying therapy for people living with classical HCU", an inherited metabolic disorder.
What to watch: the US regulatory filing to begin dosing studies for civorebrutinib, which the company calls a "pipeline-in-a-product across multiple immune-mediated kidney diseases", planned by year-end. Filing on schedule lets late-stage kidney trials start in 2027.
🐻 Bear Case
Combined Reporting Hides Whether IgAN Patients Are Leaving
Chief executive Eric Dube said the company "continued to expand our reach in IgAN and FSGS", and the release pointed to the drug's "foundational role in IgAN and encouraging early launch momentum in FSGS". Yet for a second straight quarter, Travere gave only one combined number—1,932 start forms—for both kidney diseases.
Before FSGS won approval in April, IgAN alone brought in 993 start forms in the first quarter. Meanwhile, rival kidney drugs—including Novartis's atrasentan and iptacopan, plus newer immune-targeting injections—are competing for those same IgAN patients, and insurers often resist paying for two expensive branded drugs at once.
Travere presented two small data sets at the American Society of Nephrology conference to argue doctors should keep Filspari as a base therapy, including a 12-patient first-line study and urine biomarker results showing "rapid and sustained reductions in markers of macrophage and B-cell activation, inflammation, and complement activation". Because management withholds the breakdown by disease, readers cannot tell whether IgAN starts stayed near 1,000 or dropped to 600 while 1,300 FSGS forms covered up the loss.
What to watch: any commentary on IgAN prescription trends on the conference call or in the quarterly filing. If total start forms slip toward 1,500 once early FSGS demand settles, it will signal that rival IgAN launches are taking share.
Capitalized Royalties and Thin Equity Mask Cash Costs
Travere owes Ligand Pharmaceuticals a 15% to 17% royalty on Filspari sales, plus partner milestones, and it also took on $113.1 million in upfront costs and up to $1.03 billion in future milestones for civorebrutinib. Under an accounting rule called the cost accumulation model, the company records those royalty and milestone obligations as an intangible asset on its balance sheet rather than as an immediate expense.
In the third quarter, Travere recorded $8.0 million of GAAP royalty expense and $3.2 million excluding amortization, while net intangible assets jumped by $32.6 million from June to $164.1 million. By our math, the company added $37.4 million of capitalized royalties and milestones to the balance sheet while amortizing $4.8 million through the income statement, and stockholders' equity slipped to $22.6 million against $603.5 million of convertible debt.
What to watch: the investing cash flow line in the third-quarter 10-Q filing. It will show the exact cash paid out for capitalized royalties and milestones that never touched operating profit.
Risks this quarter didn't answer
Three standing operational and financial questions received no new figures in the third-quarter press release or continued to weigh on results.
- Filspari price discounts: management previously forecast gross-to-net discounts rising to the mid-20% range this year as more Medicare FSGS patients start treatment, and the release gave no third-quarter rate.
- Tiopronin generic erosion: net sales of the older kidney stone drugs fell 10.1% from a year ago to $20.0 million, continuing a steady slide against generic copies.
- HARMONY trial enrollment count: the release said enrollment is progressing toward a 70-patient target for data in the second half of 2027, without saying how many patients have enrolled since the trial restarted in early 2026.

Bradley Campbell Takes Over as Chief Executive in December
Travere confirmed its previously announced leadership handover, with Bradley L. Campbell succeeding Eric Dube as president and chief executive officer and joining the board on December 1, 2026. Dube will stay on as an executive advisor through February 15, 2027, and work as a consultant after that date to support the transition while the company runs the FSGS launch and two late-stage drug programs.
💲 Other KPIs
Money owed by drug distributors rose 33.4% from the second quarter and 34.2% from the end of 2025, reaching $107.5 million. That increase tracked the 36.5% quarterly jump in US product sales, showing that customers are paying bills on a normal schedule as Filspari volume expands.
Cash and marketable debt securities dipped by $17.4 million from June to $471.8 million, leaving $131.7 million in net debt against $603.5 million of convertible notes by our math. Because the company paid $112.5 million in cash to Everest Medicines in July, the business generated $95.1 million in cash from operations and working capital by our math.
Weighted average basic shares rose 6.0% from a year ago and 1.2% from the second quarter to 94.6 million, driven by employee stock compensation. Because the company posted a GAAP net loss on the one-time licensing charge, potential shares from convertible notes were left out of the diluted count.
🔮 Guidance
Unchanged. The company still expects topline results from the 70-patient Phase 3 HARMONY study of pegtibatinase in classical homocystinuria in the second half of 2027, following the trial's enrollment restart in the first quarter of 2026.
Unchanged. Having completed patient enrollment in the second quarter of 2026, management continues to anticipate clinical data in 2027 from the SPARX study testing Filspari in post-transplant patients with recurring IgAN or FSGS.
Unchanged. Study startup is underway for the Phase 4 open-label SPARLIGHT trial in high-risk adult and pediatric FSGS patients, and the company plans to enroll the first participant by the end of 2026.
New. Following the July closing of the licensing deal with Everest Medicines, the company plans to submit a US Investigational New Drug application by the end of 2026 to begin dosing studies across immune-mediated kidney diseases.
❓ Key Questions
How many of the 1,932 start forms came from FSGS?
Combined reporting hides whether older IgAN prescriptions are holding above 900 per quarter or shrinking against rival kidney drugs while new FSGS patients fill the gap.
What was Filspari's gross-to-net discount rate in the third quarter?
Finance leadership previously warned that price discounts would rise in the second half as more Medicare FSGS patients start therapy, and the release gave no update on the full-year mid-20% target.
How much cash went to capitalized royalties and milestones?
Net intangible assets rose by $32.6 million in the quarter, meaning a large cash or accrued obligation to partners bypassed the income statement's royalty expense line.
How many patients are enrolled in the HARMONY trial?
Topline results for pegtibatinase are scheduled for the second half of 2027 across a 70-patient target, and investors have no count of patients enrolled since the study restarted in early 2026.
