Teladoc Health (TDOC) Q3 2026 earnings review
Teladoc Health lifts its profit, but therapy sales fall faster
Teladoc Health's virtual medical unit for employers and health plans is expanding its profit while its BetterHelp online therapy business keeps shrinking. Total sales slipped 3% from a year ago to $605 million, yet adjusted EBITDA rose 14% to $79 million as the company cut advertising spending by a quarter and chronic-care enrollment climbed. Chief executive Chuck Divita nudged the full-year sales plan higher, but the fourth-quarter outlook points to revenue falling 8% at the middle of the range.
| Revenue | $605.1M -3.4% from a year ago |
|---|---|
| Adjusted EBITDA | $79.4M +13.5% from a year ago (beat $62–74M plan) |
| BetterHelp cash-pay vs insurance revenue | -25% / $32.1M cash-pay down $58.7M YoY; insurance up $28.4M |
| Full-year revenue outlook | Raised $13.5M middle of the range: -4.4% from 2025 |
⚖️ Verdict: ⚪ Neutral
The story is unchanged because fast growth in insurance-covered therapy still replaces less than half of the cash-paying customers BetterHelp is losing. On the positive side, third-quarter free cash flow of $76 million removed the worst of the second-half cash squeeze, and the medical segment beat its margin target again. The bad news: BetterHelp's cash-pay sales drop deepened to 25%, and its fourth-quarter plan calls for total therapy sales to fall 19% to 32% from a year ago while full-year segment margin expectations were trimmed.
The question now is whether BetterHelp's shift to health insurance builds a larger therapy business or leaves it permanently smaller and less profitable. One view is that adding credentialed therapists will let insured visits overtake the cash-pay drop; the other is that lower reimbursement rates and ad cuts lock in a smaller base. First-quarter 2027 therapy revenue and margins, when seasonal marketing returns, will settle it.
🐂 Bull Case
Integrated Care Margin Hit 18.8% as Chronic Care Grew
Integrated Care is Teladoc Health's virtual medical and chronic-condition business for health plans and employers. It carries two-thirds of total revenue and almost all of the profit. This quarter the unit beat its own margin plan for a third straight time, lifting its adjusted EBITDA margin (profit before interest, taxes, depreciation, amortization and one-offs) to 18.8%, above the 15.7% to 17.2% outlook.
- Integrated Care revenue: $398.5 million, up 2.3% from a year ago
- Segment adjusted EBITDA: $74.9 million, up 13.3% from a year ago
- Chronic care program enrollment: 1.344 million members, up 15.4% from a year ago and up 72,000 from last quarter by our math
- Average monthly revenue per U.S. member: $1.33, up 4.7% from a year ago
The unit is earning more from each member because more people are enrolling in its chronic-care bundles, even though total U.S. health-plan membership dipped 2% from a year ago to 100.1 million. That prompted the company to raise its full-year margin plan for the division to 16.0%–16.6%.
What to watch: Integrated Care's adjusted EBITDA margin in the fourth quarter, which the company forecasts at 14.5% to 16.8%. Staying near the top of that range would confirm that chronic-care bundles and lower overhead can keep growing profit despite flat health-plan membership.
Marketing and Stock-Pay Cuts Drove the Profit Beat
Teladoc Health beat the top of its $62–74 million adjusted EBITDA outlook, delivering $79.4 million even as total sales fell 3.4% from a year ago. The virtual-care provider achieved this by pulling back on customer acquisition ads in its therapy unit and trimming employee stock awards. Advertising and marketing fell 6.3 percentage points of revenue from a year ago, dropping 26.2% to $124.0 million.
At the same time, stock-based compensation fell 31.7% to $11.6 million, while cost of revenue (the direct cost of delivering medical and therapy visits) rose 1.6 percentage points of revenue to 31.4%. What to watch: advertising and marketing as a share of revenue in the fourth quarter, because if cost of revenue keeps rising as lower-margin insurance visits grow, profit will rely on deeper ad cuts.
Strong Q3 Cash Flow Eased the Second-Half Target
Entering the quarter, Teladoc Health had generated only $9.4 million of free cash flow in the first half of the year, leaving a steep climb to reach its full-year plan. Third-quarter free cash flow (operating cash flow minus capital spending and capitalized software) rose 11.8% from a year ago to $75.9 million, or 12.6% of revenue, lifting the nine-month total to $85.4 million.
Management narrowed its full-year free cash flow outlook to $135–165 million while keeping the $150 million middle of the range unchanged. By our math, that leaves $64.6 million for the fourth quarter against $53.4 million a year ago. What to watch: fourth-quarter operating cash flow, because reaching the $150 million target requires collecting the $39.0 million jump in receivables built up over the first nine months.
🐻 Bear Case
BetterHelp Insurance Beat Still Replaces Half the Cash-Pay Loss
BetterHelp is Teladoc Health's online therapy service, which is shifting from customers paying out of pocket to sessions covered by health insurance. Proponents view the fast-growing insurance line as a new growth engine. Chief executive Chuck Divita said that "In the BetterHelp segment, insurance revenue exceeded our expectations, and we are making encouraging progress expanding provider capacity and scaling access to covered services."
- BetterHelp insurance revenue: $32.1 million, up 47.5% from last quarter by our math and above the $25–31 million call outlook
- BetterHelp cash-pay revenue: $174.5 million, down 25.2% from a year ago
- BetterHelp paying users: 336,000 average monthly users, down 12.0% from a year ago and down 10,000 from last quarter
- Fourth-quarter BetterHelp revenue outlook: down 31.5% to 18.5% from a year ago
Despite the insurance beat, the segment's total sales fell 12.8% to $206.6 million because the company attributes the drop to "continued actions to prioritize Insurance Covered Services and the continuing decline in users paying directly out of pocket". By our math, insurance added $28.4 million from a year ago while cash-pay lost $58.7 million, replacing only 48 cents of every lost dollar.
What to watch: fourth-quarter BetterHelp revenue against the 25.0% decline at the middle of the outlook. A drop that steep would show that cutting marketing to shift toward insurance is shrinking the therapy business faster than insured visits can rebuild it.
BetterHelp Q4 Margin Plan Rests on Steep Ad Cuts
BetterHelp earned a 2.2% adjusted EBITDA margin in the third quarter, bringing its nine-month margin to 1.1% ($6.8 million of profit on $637.7 million of sales). Even so, Teladoc Health forecasts a full-year segment margin of 2.7% to 4.3%—trimmed by 0.3 percentage points from last quarter's plan—and issued a fourth-quarter margin outlook of 9.2% to 15.1%.
Hitting that fourth-quarter jump while segment revenue falls about 25% from a year ago relies on seasonal pullbacks in therapy ad spending rather than healthier unit economics. What to watch: first-quarter 2027 BetterHelp margin outlook when the company reports next quarter, because when customer-acquisition marketing resumes in January, the margin will show what the insurance-heavy mix earns on a normal cost base.
Risks this quarter didn't answer
Five standing concerns around the balance sheet, leadership, unit economics and legal exposure received no new resolution in the third-quarter earnings release.
- $1.0 billion debt maturity: $997.6 million of convertible notes sit in current liabilities due June 2027 against $849.9 million of cash; an executed refinancing agreement would settle the risk.
- Vacant finance chief role: no chief financial officer is named a year after the prior CFO's departure was announced; naming a permanent successor would resolve it.
- Therapist capacity and insurance gross margin: the release gives neither the revenue capacity of credentialed therapists nor the gross margin gap between insurance and cash pay; disclosing either number on the call or in the 10-Q would test the pivot.
- U.S. Integrated Care membership erosion: members slipped another 200,000 from last quarter to 100.1 million (-2.3% from a year ago); the 2027 enrollment outlook will show if health-plan losses have stabilized.
- BetterHelp goodwill and litigation: $283.2 million of goodwill remains on the balance sheet after last quarter's impairment test warning, alongside pending shareholder and privacy suits; the 10-Q footnote will update both.

💲 Other KPIs
Total deferred revenue—cash customers have paid in advance for services not yet delivered—fell 12.6% from a year ago ($62.0 million current plus $9.1 million long-term, vs $81.4 million a year ago), though it ticked up $0.6 million from last quarter. It is shrinking because insured therapy bills after each completed visit rather than collecting monthly cash subscriptions upfront.
Accounts receivable—money billed to health plans and clients that has not yet been collected in cash—rose 9.6% from a year ago and is up $38.1 million since December, even as total revenue fell 3.4%. The buildup reflects a growing mix of visit-based and insurance-billed revenue (Other revenue rose 31% to $138.4 million while access fees fell 10%).
The share count used to calculate net loss per share rose 2.8% from a year ago (from 176.9 million) and increased by 850,000 shares from the second quarter. Teladoc Health has not repurchased stock while conserving cash for its 2027 debt maturity, though lower employee stock grants have slowed the pace of new share issuance.

Cut. The full-year margin range dropped by 0.3 percentage points at both ends, putting the middle of the plan at 3.5% versus 4.4% delivered in 2025.
New. The fourth-quarter outlook calls for a 12.15% margin at the middle of the range, up from 2.2% in the third quarter and 7.9% in the fourth quarter of 2025 as seasonal marketing cuts take hold.
❓ Key Questions
What revenue can BetterHelp's credentialed therapists support today?
Chief executive Chuck Divita cited progress expanding provider capacity, which capped insurance visits last quarter. Without knowing how much annual revenue the current therapist network can handle, investors cannot tell when insurance growth will catch up with the $58.7 million quarterly drop in cash-pay revenue.
How many percentage points lower is the insurance gross margin?
Cost of revenue rose 1.6 percentage points of revenue from a year ago as BetterHelp insurance sales reached $32.1 million. Management has said insured therapy carries a lower gross margin than cash pay, but has never disclosed the exact gap.
How will Teladoc Health refinance the $998 million June 2027 notes?
The 1.25% convertible notes are now a current liability due in seven months, and the $849.9 million cash balance does not cover the full principal on its own. Investors need the size, interest rate and covenants of any new term loan or credit draw.
What happens to BetterHelp margins when ad spending returns in Q1?
The fourth-quarter plan relies on a seasonal pullback in advertising to lift BetterHelp's adjusted EBITDA margin from 2.2% to a range of 9.2%–15.1%. The question is whether the segment stays profitable in the first quarter when customer-acquisition spending normally restarts.
