uniQure (QURE) Q3 2026 earnings review
FDA fast-tracks uniQure's Huntington's drug, but four-year data weakens
uniQure, a developer of one-time gene therapies, cleared its main regulatory hurdles for Huntington's disease while posting mixed four-year trial results. The US regulator accepted the AMT-130 application under Priority Review for a May 2027 decision and agreed on a 200-patient follow-on study. However, in 12 high-dose patients tracked for four years, disease slowing on the main composite scale dropped to a non-significant 44%, even as the three-year group showed 80% slowing.
| AMT-130 FDA decision date | May 1, 2027 Priority Review granted Nov 2026 |
|---|---|
| 48-month cUHDRS slowing | 44% non-significant (p=0.144) in 12 patients |
| Cash and investments | $768.4M -$42M from last quarter; funds into 2030 |
| Overhead and commercial spend | $23.9M +23% from a year ago |
โ๏ธ Verdict: ๐ข Bullish
The story got better because the FDA accepted the filing, granted a six-month review, and settled the confirmatory trial design that had been open for three quarters. In addition, the Fabry disease trial resumed dosing after liver safety issues resolved. One caution: commercial hiring lifted overhead spending as cash fell to $768 million, and the non-significant four-year composite result gives reviewers a clear target.
The question now is whether the FDA grants accelerated approval on the strong three-year data and four-year functional scores, or balks because the composite scale lost statistical significance at four years. If reviewers focus on the functional endpoint used in the new 200-patient trial, the drug clears; if they require four-year consistency across scales, the application stalls. The FDA decision on May 1, 2027 will settle it.
๐ Bull Case
FDA Sets May 2027 Decision Date
uniQure develops one-time gene therapies, led by AMT-130 (ifezuntirgene inilparvovec) for Huntington's disease, an inherited brain disorder. In November, the US Food and Drug Administration (FDA) accepted the company's application for accelerated approval and granted Priority Review, which shortens the review clock to six months and sets a decision target of May 1, 2027.
In the third quarter, the UK regulator also validated uniQure's marketing application. Because the US agency had rejected the same three-year dataset earlier in 2026 before reversing course in July, formal acceptance removes the risk that regulators would refuse to review the file.
What to watch: the quarterly filing or the next earnings call for whether the FDA schedules an outside advisory committee meeting before May 1, 2027. A scheduled panel would put the small 12-patient dataset under public scrutiny before any approval.
FDA Agrees on 200-Patient Trial Design
To keep an accelerated approval, a drugmaker must run a follow-on confirmatory trial. For three quarters, uniQure had no agreed design or size for this study. It now has an agreement with the FDA for a "randomized, standard-of-care-controlled trial enrolling approximately 200 patients".
Patients will be "enrolled in a 2:1 ratio to treatment or standard of care, with TFC as the primary endpoint at 36 months" โ Total Functional Capacity, a measure of daily functioning. Patient screening in the US starts by the end of 2026, which clears a precondition for approval and avoids sham brain surgery for the control group.
What to watch: how many patients are screened and enrolled by the May 1, 2027 decision date. Slow enrollment would test the FDA's expectation that the study be well underway at the time of approval.
Fabry Trial Resumes Dosing after Pause
Beyond Huntington's disease, the biotech group is testing AMT-191 for Fabry disease, an inherited enzyme deficiency, and AMT-260 for drug-resistant epilepsy. Dosing in the middle and high doses of AMT-191 had been paused since late 2025 after "asymptomatic liver enzyme elevations in two mid-dose patients, which resolved following a course of immunosuppression".
In August, uniQure resumed enrolling both cohorts, and in the third quarter it finished enrolling the seven-patient second cohort for AMT-260. Both trials will report updated results in the first half of 2027, giving the early-stage pipeline two clinical readouts around the time of the Huntington's decision.
What to watch: liver safety data across all AMT-191 dose cohorts in the first half of 2027. Any recurrence of high liver enzymes would threaten the program's viability.
๐ป Bear Case
Four-Year Data Misses Primary Measure
"We strongly believe AMT-130 has the potential to deliver meaningful benefit to HD patients," chief executive Matthew Kapusta said. The drug was "generally well tolerated with a manageable safety profile", and the US filing rests on 36-month data where 15 high-dose patients showed an 80% slowing on the composite Unified Huntington's Disease Rating Scale (cUHDRS).
In September, uniQure released 48-month results for 12 high-dose patients against an external control group. On cUHDRS, the pre-specified main measure for that analysis, disease slowing dropped to 44% and missed statistical significance (p=0.144).
- cUHDRS slowing at 36 months (15 patients): 80% (nominal p=0.005)
- cUHDRS slowing at 48 months (12 patients): 44% (non-significant p=0.144)
- Total Functional Capacity slowing at 48 months: 61% (nominal p=0.008)
This matters because the FDA review will examine all available data. While the functional scale (TFC) held up at four years and became the primary measure for the confirmatory trial, a non-significant result on the composite scale at four years gives FDA reviewers a clear point of pushback.
What to watch: FDA briefing documents ahead of the May 1, 2027 target date. They will show whether agency statisticians weight the three-year cut or the weaker four-year cut.
Commercial Hiring Lifts Overhead Before Trial Ramp
uniQure is building its sales and medical teams while preparing to fund the 200-patient confirmatory trial. The company holds $768.4 million in cash and investments, which it says will fund operations into 2030. That balance fell by $41.9 million this quarter by our math, even before patient screening begins.
Selling, general and administrative expenses โ overhead and commercial preparation โ rose 23.0% from a year ago to $23.9 million (and jumped 37.7% from last quarter) "to support the potential commercial launches of ifezuntirgene inilparvovec". By our math, lasting until 2030 allows an average draw of about $55 million a quarter, leaving limited room once a 200-patient neurosurgery trial starts.
What to watch: quarterly cash draw in the first half of 2027 as US trial screening turns into surgical dosing. A quarterly drop above $55 million would mean the cash will not last until 2030 without new borrowing or share sales.
Royalty Debt Grows Faster Than Hemgenix Sales
The company's only product revenue comes from licences and royalties on Hemgenix, a hemophilia B gene therapy sold by partner CSL Behring. Because uniQure sold the rights to those royalties in 2023, the payments go toward a financing liability rather than to the company's usable cash.
In the third quarter, total revenue was $5.2 million, while the non-current royalty liability on the balance sheet rose by $7.9 million by our math to $497.3 million. Because accrued interest on the obligation is larger than the royalty payments coming in, the debt compounds every quarter.
What to watch: the royalty financing footnote in the third-quarter 10-Q filing. It will show the exact interest added, the royalties paid out, and whether management lowered its long-term sales forecast again.
Risks this quarter didn't answer
Three standing concerns from prior quarters received no update in the third-quarter press release.
- Confirmatory trial budget: uniQure disclosed the 200-patient size, but still has not stated the dollar cost embedded in its 2030 cash plan.
- UK reimbursement path: the UK regulator validated the application, but management gave no timeline for coverage talks with the National Health Service.
- Shareholder lawsuit: the release did not mention the status of the securities class action, where a motion to dismiss was due in September 2026.
๐ฒ Other KPIs
License revenue rose 40.4% from a year ago, when the company recorded $3.7 million, but fell 11.0% from the second quarter's $5.8 million. Cost of license revenues fell to $0.2 million from $0.4 million a year earlier, leaving most of the top line to flow through to operating results.
Research spending increased 3.3% from a year ago and 4.6% from last quarter. Employee and contractor costs rose $3.6 million, and spending on the AMT-260 epilepsy trial and early projects grew by $3.2 million, which more than offset a $4.1 million drop in manufacturing validation work for AMT-130.
The share count used to calculate loss per share rose 19.5% from a year ago and 5.5% from the second quarter. The increase reflects the June 2026 share offering and the exercise of pre-funded warrants, which reduced the warrant liability on the balance sheet from $24.2 million in June to zero.
๐ฎ Guidance
Unchanged. Management still expects its $768.4 million in cash and investment securities to fund operations into 2030. By our math, that 14-quarter window implies an average cash draw of about $55 million per quarter, up from the $41.9 million reduction in the third quarter as the 200-patient confirmatory trial and commercial launch costs ramp up.
New. Following the third-quarter application filing, the FDA granted Priority Review and set a target decision date of May 1, 2027. In tandem, the company plans to begin screening US patients for the 200-patient confirmatory trial by the end of 2026, with a primary readout at 36 months.
Unchanged. The gene therapy maker still plans to present updated clinical results from all dosing cohorts in the Phase I/II study of AMT-191 for Fabry disease and from the Phase I/IIa study of AMT-260 for epilepsy in the first half of 2027.
โ Key Questions
Why did cUHDRS lose significance at 48 months?
Disease slowing on the composite scale dropped from 80% at 36 months (15 patients) to a non-significant 44% at 48 months (12 patients). Did the three patients not yet at four years drive the difference, or did the treatment effect wane in year four?
What will the 200-patient confirmatory trial cost?
uniQure agreed with the FDA on a 200-patient neurosurgery study tracked for 36 months, but has not shared the budget. How much of the $768.4 million cash balance is allocated to this trial through 2030?
How many patients must be enrolled by May 1, 2027?
US screening starts by the end of 2026, just four months before the FDA's May 1, 2027 decision date. How many patients does the FDA expect uniQure to have screened or dosed before granting accelerated approval?
Will the FDA hold an advisory committee meeting?
Management previously expected an outside panel review. Now that the application is under a six-month Priority Review, has the FDA confirmed whether it will convene an advisory committee, and on what date?
