Precigen (PGEN) Q3 2026 earnings review

Precigen turns profit into cash early, while manufacturing costs rise

Precigen, the maker of the throat-disease immunotherapy Papzimeos, started collecting customer bills and turned cash-flow positive ahead of schedule. Drug revenue rose 37% from the second quarter to $73 million, lifting net profit to $36 million and generating $12 million in operating cash flow. Chief executive Helen Sabzevari said well over 500 patients have now started the therapy, more than double the treated count from three months ago.

At a glance
Papzimeos net revenue$72.9M +37% from last quarter
Operating cash flow$11.9M reached positive cash flow a quarter early
Consolidated gross margin91.2% down 3.7 percentage points from last quarter
Patients completing treatmentOver 300 up from over 100 last quarter

โš–๏ธ Verdict: ๐ŸŸข Bullish

The story got better because customer payments finally arrived and settled the biggest balance-sheet risk. Generating positive operating cash flow a full quarter ahead of its year-end plan lifted total cash and investments to $51 million, proving the 127-day billing cycle works. One caution: gross margin dropped nearly 4 percentage points as zero-cost pre-approval inventory ran down, and the release omitted the patient hub registration count.

The question now is whether quarterly sales keep climbing once the initial backlog of severe patients finishes treatment. Either community doctors drive steady new demand among milder patients, or sales level off as the one-course therapy cures its early adopters. Fourth-quarter patient start numbers and revenue on the next call will tell.

๐Ÿ‚ Bull Case

concern eased CASH

Customer Payments Turned Cash Flow Positive Early

Precigen sells Papzimeos, its immunotherapy for recurrent respiratory papillomatosis (RRP, a rare viral throat disease), on 127-day payment terms. Those four-month terms drained cash during the first half of the year and raised liquidity concerns. "We previously guided to positive cash flow by the end of the year, and we are pleased to have achieved it a full quarter ahead of that guidance, with net cash provided by operating activities of $11.9 million," finance chief Harry Thomasian Jr. said.

By our math, the biotechnology company collected about $49.5 million from customers during the third quarter as bills from the spring came due. That cash inflow lifted total cash and investments by $12.3 million to $51.0 million, easing the balance-sheet pressure against its $94.3 million debt load.

What to watch: fourth-quarter operating cash flow and the minimum liquidity rules in the quarterly filing. Collecting the $71.9 million of June bills during the fourth quarter would confirm that cash generation is now self-sustaining.

๐ŸŸข๐ŸŸข strengthening GROWTH

Treated Patient Counts More Than Doubled

Papzimeos is the first approved drug for adults with RRP, replacing repeated throat surgeries with a four-dose course of injections. "PAPZIMEOS has transformed treatment for adults with RRP, with well over 500 patients initiating treatment and 300 of them having completed treatment," chief executive Helen Sabzevari said. Three months ago, those figures stood at over 200 patients started and over 100 completed.

  • Papzimeos net revenue: $72.9 million, up 37.3% from last quarter
  • Patients starting treatment: well over 500, up by at least 300 from last quarter by our math
  • Patients finishing all four doses: over 300, up by at least 200 from last quarter by our math
  • US insurance coverage: over 315 million lives, or nearly all insured Americans

This jump matters because doctors are now prescribing the drug across community clinics and "including among patients with fewer than three surgeries in the preceding year". Broadening beyond the sickest hospital patients expands the active market, though sequential revenue growth slowed from 145.8% in the second quarter as the revenue base grew larger.

What to watch: fourth-quarter Papzimeos revenue and new patient starts on the conference call. Steady additions would show that community adoption is replacing the initial wave of severe patients.

๐ŸŸข strengthening MARGIN

Operating Margin Crossed 50% on Flat Overhead

Precigen built its commercial sales force before launching Papzimeos last August, so new sales now flow directly to profit. Operating margin rose 10.9 percentage points from last quarter to 52.1%, generating $38.7 million in operating income and $35.8 million in net profit.

Sales and administrative overhead stayed flat at $22.4 million, which dropped from 40.5% of revenue in the second quarter to 30.2% in the third by our math. Research and development spending also fell by 4.3 percentage points of revenue to 8.9%, allowing revenue growth to outpace total expenses.

What to watch: quarterly sales and administrative expenses in the fourth quarter. Holding overhead near $22 million while sales grow would keep operating margins above 50% even as manufacturing costs rise.

๐Ÿป Bear Case

๐Ÿ”ด persistent MARGIN

Gross Margin Dropped as Free Inventory Depleted

Before winning regulatory approval in August 2025, Precigen recorded the cost of making Papzimeos as a research expense. That accounting rule let the drugmaker sell its early batches at nearly 100% gross margin during the first three quarters of the launch.

Those zero-cost batches began running out this quarter. Cost of products and services more than doubled from $2.8 million in the second quarter to $6.5 million in the third, rising by 3.7 percentage points of revenue to 8.8% by our math. As a result, consolidated gross margin stepped down from 94.9% to 91.2%.

What to watch: fourth-quarter gross margin once all pre-approval batches are gone. A reading below the high-80s percentage range that Thomasian previously outlined would mean ongoing manufacturing costs are higher than planned.

๐Ÿ”ด persistent PRODUCT contradicts narrative

Research Spending Fell Despite Pipeline Growth Plans

Sabzevari said the company is advancing a lifecycle plan for Papzimeos with an "ongoing redosing clinical trial, pediatric development, and geographic expansion activities", alongside Phase 2 trials for PRGN-2009, its experimental immunotherapy for HPV-related head and neck and cervical cancers. Previously, management also stated in its filings that research and development expenses would increase as 2026 progressed.

The numbers moved the other way. Research and development spending fell 8.9% from the second quarter to $6.6 million, and dropped 46.4% from a year ago. In addition, the company did not set a date in the release for its promised year-end PRGN-2009 clinical data readout, saying only that it will present the data "at a forthcoming R&D Day".

What to watch: the date of the R&D Day and whether the pediatric RRP trial begins before year-end. Pushing either milestone into 2027 would leave Precigen reliant on a single adult indication for longer.

๐Ÿ”ด persistent DISCLOSURE

Risks this quarter didn't answer

Several standing concerns about patient funnel visibility and long-term market size got no new data in the release.

  • Patient hub registrations: omitted from the press release after sitting at over 500 last quarter; the conference call or quarterly filing will show if new registrations slowed.
  • Single-product exhaustion: five-year durability data means cured patients do not return; expanding the pool requires results from the ongoing redosing trial and the European regulatory review.
  • Revenue outlook policy: management still provides no quarterly or annual sales targets, leaving investors to estimate future demand from cumulative patient milestones.

FDA Granted Platform Status to AdenoVerse

In September 2026, the Food and Drug Administration granted Platform Technology Designation to AdenoVerse, the viral delivery technology behind Papzimeos and PRGN-2009. The status allows the company to reuse manufacturing and safety data from Papzimeos when filing new drug candidates, which can speed up future clinical trials, though it has no immediate impact on revenue.

๐Ÿ’ฒ Other KPIs

Trade receivables, net (26Q3) $96.5 million
โ‡— accelerating

Uncollected customer bills rose 34.3% from $71.9 million last quarter, matching the 34.9% rise in total revenue. Because customers have 127 days to pay, receivables grow alongside sales, though the third-quarter increase ($24.6 million) was smaller than the second-quarter jump ($45.5 million) as early bills finally turned into cash.

Inventory (26Q3) $22.9 million
โ‡— accelerating

Stockpiled drug supply rose 13.2% from $20.2 million in the second quarter and more than doubled from $9.6 million at the end of 2025. Since winning approval in August 2025, the drugmaker records manufacturing costs on its balance sheet as inventory rather than expensing them as research.

Diluted share count (26Q3) 421.2 million
โ‡˜ decelerating

Diluted shares rose 37.1% from a year ago and 1.8% from the second quarter. Last year's preferred stock conversion and dilutive warrants expanded the share count, which cut third-quarter profit per share from $0.10 basic to $0.08 diluted.

๐Ÿ”ฎ Guidance

Operating cash flow break-even Achieved in Q3 2026 ($11.9 million)
๐Ÿ … raised from By the end of 2026
โ‡— accelerating

Raised and achieved early. After four straight quarters of targeting cash flow break-even by the end of 2026, Precigen generated $11.9 million of operating cash flow in the third quarter. Thomasian expects continued revenue momentum while maintaining spending discipline, though the release issued no numeric sales or profit targets for the fourth quarter.

โ“ Key Questions

How many patients registered in the hub this quarter?

Management highlighted well over 500 treated patients, the same number it gave for total hub registrations three months ago. Without an updated hub count or new quarterly starts, investors cannot tell if the forward patient pipeline is still growing.

Have pre-approval Papzimeos batches completely sold out?

Finance chief Harry Thomasian Jr. previously expected zero-cost pre-launch inventory to run out in the third quarter, taking gross margin to the high-80s or low-90s percentage range. Investors need to know how much of the 91.2% third-quarter margin still came from those early batches.

Will the PRGN-2009 data and pediatric trial happen in 2026?

Management previously planned to start a pediatric RRP trial and present Phase 2 head and neck cancer data for PRGN-2009 before year-end. The third-quarter release mentions neither the pediatric start nor a 2026 date for the forthcoming R&D Day.

What share of Q3 revenue came from Q2 patient starts?

Because a treatment course spans four doses over 12 weeks, patients who started in the second quarter received their final doses in the third. Separating carryover doses from new starts shows true underlying demand.
