Nyxoah (NYXH) Q3 2026 earnings review
Nyxoah's US implant sales rise, but it cuts full-year plan
Nyxoah, which makes the Genio chin implant for sleep apnea, grew US sales faster this quarter but cut its full-year revenue plan. Global sales rose 19% from the second quarter as the US sales team reached 233 active hospital centers, and higher factory output lifted gross margin to 63%. However, new chief executive Liam Kelly lowered the full-year revenue target to €33–35 million, which by our math points to about €11 million in the fourth quarter.
| Global net revenue | €9.2M +19% from last quarter |
|---|---|
| US net revenue | €6.7M +29% from last quarter |
| Gross margin | 63.1% 60.5% a year ago |
| Full-year revenue outlook | Cut €4M middle of the range: €34M (was €38M) |
⚖️ Verdict: 🔴 Bearish
The story got worse because the steep second-half US sales ramp management promised all year will not happen, while quarterly cash burn stayed near €21 million. The backlog of US patients in insurance review grew 5% from last quarter to 447, down from 77% growth the quarter before, and cash on hand covers less than four quarters at the current burn rate by our math. The good news: Nyxoah filed with the US regulator to expand Genio's label to circular-throat-collapse patients, and Medicare raised 2027 procedure payments by 11% to 13%.
The question now is whether the 55 US sales reps can get existing surgery centers to implant Genio regularly enough to lower cash burn before the company needs another dilutive funding round. Optimists point to the 233 active centers and higher 2027 Medicare pay; skeptics point to the stalling insurance queue and falling revenue per queued patient. Fourth-quarter US sales and cash burn will show which path the launch is on.
🐂 Bull Case
FDA Filing Targets Untreated Sleep Apnea Patients
Nyxoah sells Genio, a battery-free chin implant that stimulates both branches of the tongue nerve to keep a patient's airway open during sleep. Competing implants stimulate only one side and cannot treat patients with complete concentric collapse (CCC), where the throat closes in a full circle. Chief executive Liam Kelly noted that "CCC patients account for about one in four of the patients evaluated for this therapy and have no hypoglossal nerve stimulation option today in the United States."
During the third quarter, the Belgian medical device maker reported results from its US clinical trial in 47 CCC patients and filed an application with the US Food and Drug Administration to add those patients to Genio's label.
- Breathing interruptions: 77.2% of trial patients met the main reduction goal at 12 months
- Blood oxygen drops: 87.7% of patients met the oxygen recovery goal at 12 months
- Safety: zero serious adverse events related to the device or surgery through 12 months
An approval would give surgeons a reason to choose Genio without first running a sedated throat-camera test to rule out circular throat collapse, because Genio could treat both groups.
What to watch: the FDA decision on the CCC label expansion, which management previously timed for late first quarter or early second quarter of 2027. Approval would open a quarter of the US patient pool where Genio faces no rival nerve-stimulation implant.
US Accounts Beat Target and 2027 Pay Rose
Nyxoah is building a US sales team to win surgery centers and train ear, nose and throat surgeons on Genio. Last quarter, management aimed to grow from 40 sales representatives to 55 and reach 225 to 230 active US hospital accounts by year-end.
The company beat its year-end hospital target a quarter early and received higher final 2027 procedure payment rates from the US Medicare agency (CMS). "Today, we have 55 sales reps with 233 active accounts at the end of the third quarter," Kelly said.
- Active US accounts: 233, up by 53 accounts from last quarter
- Trained US surgeons: 342, up by 80 surgeons from last quarter
- 2027 Medicare hospital outpatient payment: $35,072, up 11% from $31,526
- 2027 Medicare surgery center payment: $31,283, up 13% from $27,563
Higher facility payments increase the profit hospitals and surgery centers keep when they implant Genio, so centers have a stronger financial reason to schedule the procedure in 2027.
What to watch: fourth-quarter US revenue per active account, which stood at €28.7 thousand this quarter by our math. A rise would show that the 55 territories are getting existing centers to perform repeat surgeries rather than just shipping starter kits to new sites.
Higher Factory Volume Lifted Gross Margin
Nyxoah struggled with manufacturing yields in the first quarter of 2026, which pushed gross margin down to 57.1% and left first-half gross margin at 58.5%, below its 60% to 62% full-year target.
In the third quarter, gross margin rose to 63.1%, up 2.6 percentage points from a year ago and 3.4 percentage points from the second quarter. Cost of goods sold — the direct cost of making and shipping the Genio implants — fell by 2.6 percentage points of revenue from a year ago to 36.9%. The company attributed the gain to "operating leverage on manufacturing-related costs as volumes increase".
That recovery lifted nine-month gross margin to 60.3% by our math, putting the business back inside its full-year target range even before its planned next-generation disposable patch arrives.
What to watch: fourth-quarter gross margin against the 62.5% rate implied by the middle of the full-year outlook by our math. Holding above 62% would confirm that factory unit costs are falling steadily as implant volumes grow.
🐻 Bear Case
Revenue Plan Cut as Insurance Queue Stalls
"We delivered 29% sequential U.S. revenue growth in the third quarter, an acceleration from 22% in the second quarter," Kelly said. Yet on October 19, Nyxoah cut its full-year global revenue outlook from €36–40 million to €33–35 million. Kelly explained that "The new guidance is based on U.S. revenues that are more reflective of historical ramps for a new technology, as physicians and centers build experience with Genio."
The forward indicators show why the old plan broke. Before surgery, US patients enter a prior-authorization queue to get insurance approval, which management previously said takes one to three months to turn into an implant.
- Patients in insurance review entering Q4: 447, up 4.7% from 427 entering Q3 (after jumping 77.2% the quarter before)
- Q3 US revenue per patient entering Q3 in review: €15.7 thousand by our math, down from €21.6 thousand in Q2 and €36.7 thousand in Q1
- Q3 US net revenue: €6.7 million, leaving €10.8 million of global revenue for Q4 at the middle of the new plan by our math
Because the insurance queue barely grew and each patient in the queue converted into less revenue within the quarter, opening 53 new hospital accounts did not produce the second-half jump the prior plan needed.
What to watch: fourth-quarter US net revenue and the number of patients in prior authorization entering 2027. Another quarter of single-digit growth in the insurance queue would mean existing centers are not refilling their surgical schedules after their first few patients.
Cash Burn Leaves Under Four Quarters of Funds
Nyxoah raised €76.1 million in net equity proceeds and drew €13.8 million in European Investment Bank debt during the second quarter to fund its US rollout. Management said in the third-quarter filing that its liquidity can cover operations for at least 12 months, and interim finance chief Loïc Moreau pledged to reduce cash burn from quarter to quarter.
Even so, cash, cash equivalents and short-term financial assets fell by €20.7 million during the third quarter to €77.1 million. At that quarterly burn rate, the balance lasts 3.7 quarters by our math.
- Free cash flow in Q3: negative €21.4 million by our math, compared with negative €20.7 million a year ago
- Weighted average common shares in Q3: 100.9 million, up 76.9% from last quarter and up 169.4% from a year ago
- Convertible bond debt: €16.7 million carrying value (€23.1 million fair value), with €1.9 million principal instalments plus interest converted into 1.77 million new shares in August at 90% of the market price
The company also filed a new risk factor report (Exhibit 99.3) warning that if its shares close below $1.00 for 30 consecutive trading days, Nasdaq will issue a delisting deficiency notice, which could hurt its ability to raise equity capital.
What to watch: fourth-quarter cash and financial assets against the €20.7 million third-quarter burn. If quarterly cash burn does not fall below €18 million, Nyxoah will need additional funding before the middle of 2027.
Both Top Roles Changed Ahead of Outlook Cut
In June 2026, Nyxoah's board decided to replace chief executive Olivier Taelman with a US-based leader to run the American commercial launch. The company appointed former Teleflex chief executive Liam Kelly starting October 1, 2026.
One day later, on October 2, chief financial officer John Landry stepped down "to pursue another professional opportunity", and international president Loïc Moreau stepped in as interim finance chief. Seventeen days after that, on October 19, the new team pre-announced third-quarter revenue and cut the full-year 2026 revenue outlook by €4 million at the middle of the range. In the third-quarter release and call, management also stopped repeating its full-year targets for total operating expenses (€99–102 million) and cash operating expenses (€88–90 million).
What to watch: the appointment of a permanent chief financial officer and the 2027 outlook on the fourth-quarter call. Reinstating an explicit operating-expense ceiling would show whether the new leadership has locked in its 2027 spending plan.
Risks this quarter didn't answer
Three standing business risks and one commercial watch item received no new numbers or resolution on the third-quarter call.
- Inspire Medical patent lawsuit: the consolidated Delaware case over three Inspire patents and three Nyxoah patents is in discovery with no loss accrued; nine-month SG&A legal fees reached €3.1 million, up from €1.2 million a year ago.
- Permanent US procedure coding and rival entry: management gave no update on the industry's 2029 billing-code application, Inspire's separate code bid, or LivaNova's expected 2027 market entry.
- Genio share inside active US centers: management did not update the ~15% average procedure share in active US accounts that it cited last quarter; next quarter's call is the next test.
- Next-generation Genio 2.2 patch and US device component removal: management did not mention either cost-reduction milestone on the call.
💲 Other KPIs
Sales outside the United States grew 42.5% from a year ago and held steady (+0.1% by our math) from the second quarter's €2.48 million. Germany generated €1.67 million (+60.0% from a year ago), while the Netherlands added €221 thousand after launching late last year. Management previously indicated second-half international sales would roughly match the first half's €4.60 million.
The quarterly operating loss narrowed by 10.3% from €24.37 million a year ago, though it widened slightly from €20.59 million in the second quarter. Research and development spending fell 8.8% from a year ago to €11.78 million as clinical trial work wound down, while selling, general and administrative costs rose 26.1% to €16.02 million to pay the 55-person US sales force.
Uncollected customer bills rose 14.6% from €6.85 million at the end of June by our math, growing slower than the 19.5% quarterly rise in global revenue. Since year-end 2025, rising trade receivables and other customer balances have absorbed €3.26 million of operating cash as US hospital billing scaled up.
🔮 Guidance
Cut. The middle of the full-year revenue plan dropped by €4 million to €34 million, which still represents 239% growth over 2025 by our math. With €23.2 million booked in the first nine months, the new plan leaves €9.8 million to €11.8 million for the fourth quarter (€10.8 million at the middle, or 17.6% growth from the third quarter by our math). Kelly said the lower target reflects a more typical adoption pace as US surgeons build experience.
Unchanged. Nyxoah still plans for full-year gross margin to land between 60% and 62%. Because nine-month gross margin reached 60.3% after the third quarter's jump to 63.1%, the middle of the full-year outlook points to a fourth-quarter gross margin of about 62.5% by our math, achievable if third-quarter factory volumes hold.
❓ Key Questions
Why did the US insurance pipeline almost stop growing?
Patients in prior authorization rose by 20 to 447 (+4.7%) in the third quarter after rising by 186 (+77.2%) in the second quarter, even though Nyxoah added 53 active hospital accounts. Management did not explain whether fewer patients are entering the queue or approvals are clearing faster.
What is the full-year operating expense plan now?
Last quarter, management guided full-year total operating expenses to €99–102 million and cash operating expenses to €88–90 million. Neither figure appeared in the third-quarter release or call remarks after the finance chief stepped down.
What is Genio's procedure share inside active US centers?
Last quarter, management said Genio captured about 15% of nerve-stimulation implants in its active US centers, with top accounts above 40%. With the strategy shifting from opening accounts to driving repeat usage, investors need that utilization reading.
Is the Genio 2.2 patch still scheduled for early 2027?
Management previously said removing an internal component in late 2026 and launching the lower-cost Genio 2.2 disposable patch in early 2027 would push gross margin above 70%. Neither milestone was mentioned this quarter.
