electroCore (ECOR) Q3 2026 earnings review

electroCore narrows its operating loss, but cuts its sales outlook

electroCore, a maker of handheld nerve-stimulation devices, cut its operating losses sharply while its revenue growth slowed. Third-quarter sales rose 12% from a year ago to $9.7 million, as a 38% drop in consumer wellness sales offset steady prescription demand in Veterans Affairs hospitals. Co-chief executive Joshua Lev held operating expenses below last year's level, narrowing the Adjusted EBITDA loss by 67% to $0.7 million.

โš–๏ธ Verdict: ๐Ÿ”ด Bearish

The story got worse because top-line momentum broke enough to force a full-year outlook cut and another delay to the profitability timeline. Management lowered its 2026 revenue plan from over 30% growth to about 26% at the middle of the range, and Quell sales shrank 8% from the second quarter. On the positive side, new sales reps lifted revenue in their territories by 30% and gross margin reached 88.5%.

The question now is whether the commercial overhaul can lift quarterly revenue fast enough to stop the cash drain before debt repayments begin in 2027. Bulls point to the 30โ€“40% preliminary growth plan for 2027 and lower sales overhead; skeptics note that cash is down to $7.5 million and the breakeven target just slipped to the end of 2027. Fourth-quarter sales against the implied $11.8 million target will tell.

๐Ÿ‚ Bull Case

Lower Overhead and Product Mix Narrowed Operating Losses

electroCore makes handheld devices that stimulate nerves through the skin to treat headaches and pain. Co-chief executive and finance chief Joshua Lev said the quarter showed "the early stages of a more efficient commercial organization" and produced the smallest operating loss in company history. Operating lossExcluding one-offs (Adjusted EBITDA loss) narrowed 67% from a year ago to $0.68 million.

  • Selling, general and administrative costs (sales and overhead): 100.1% of revenue, down 11.5 percentage points from a year ago by our math
  • Research and development costs: 4.4% of revenue, down 3.2 percentage points by our math
  • Gross margin: 88.5%, up 2.5 percentage points on a higher share of prescription sales

Total operating expenses fell 2% even as sales grew 11.5%, because the bioelectronic device maker cut legal and professional fees and left research roles open. Note that reported net loss of $1.34 million also benefited from $0.53 million in one-off legal settlement and UK tax gains, leaving the underlying net loss at $1.87 million by our math.

What to watch: variable sales commissions as a share of revenue in the third-quarter filing. A drop toward the 27% target would show the new pay plan is saving money alongside the overhead cuts.

๐ŸŸข strengthening GROWTH

New Veterans Affairs Reps Lift Territory Sales 30%

The company sells its flagship gammaCore headache device and Quell pain wearable primarily through the U.S. Department of Veterans Affairs (VA). In the second quarter, management split its sales map into smaller regions and hired independent commission-only representatives (1099s) to increase accountability. In October, it hired two Area Vice Presidents and two Regional Sales Directors in what it called the "last material changes to the commercial organization".

Those newly trained reps averaged $210,000 a month in VA sales during the third quarter, up 30% from the $160,000 those territories generated before. New patient starts per hospital account reached 3.4 by the third month, compared with 0.3 for earlier reps. Meanwhile, the share of orders moving through federal distributor Lovell Government Services rose from 57% in June to 91% in September, which management expects will cut administrative fees by 3%.

What to watch: whether the top 30 VA accounts drop below their current 71% share of VA sales in the fourth quarter. A lower share would confirm that new reps are opening productive new hospitals rather than relying on existing ones.

๐Ÿป Bear Case

๐Ÿ”ด๐Ÿ”ด new GROWTH contradicts narrative

Sales Slowdown Forces a Full-Year Outlook Cut

"We are deliberately prioritizing profitable, sustainable long-term growth over short-term results," Lev said. Three months ago, the medical device group raised its full-year sales outlook to more than 30% growth, which by our math required at least $41.6 million for the year and about $10.9 million in the third quarter.

Instead, third-quarter revenue reached $9.69 million, up 11.5% from a year ago and down from 28.0% growth in the second quarter. Management cut the full-year outlook to $39โ€“42 million, or 26.4% growth at the middle of the range by our math. Consumer wellness revenue โ€” mostly the non-prescription Truvaga stress device โ€” fell 38.1% to $1.04 million as the company pulled back on ad spending. Even stripping out a one-time $0.5 million clinical trial order a year ago, wellness sales fell 12.0% by our math.

What to watch: fourth-quarter total revenue against the $11.8 million needed to hit the middle of the new full-year range. Reaching that figure requires a 21.5% jump from the third quarter, which will test whether prescription growth can offset the smaller consumer business.

Quell Sales Shrank Sequentially and Breakeven Slipped Again

Quell is a wearable nerve stimulator for fibromyalgia pain that electroCore bought from NeuroMetrix in May 2025. Until this quarter, Quell was the company's fastest-growing product line, climbing from $0.60 million in the third quarter of 2025 to $1.0 million in the first quarter of 2026 and $1.30 million in the second quarter.

In the third quarter, Quell sales slipped 7.7% from the prior quarter to $1.20 million, with $1.10 million coming from the VA. By our math, U.S. prescription sales outside Quell VA grew 17.8% from a year ago to $6.85 million, up from an 11% pace in the second quarter, so gammaCore picked up some slack. However, management also pushed back its profitability goal: it now expects to "achieve its first quarter with positive Adjusted EBITDA by the end of 2027", compared with its target three months ago of the third quarter of 2027.

What to watch: fourth-quarter Quell revenue. A rebound above $1.3 million is necessary to show the third-quarter dip was temporary order timing rather than early saturation in current VA clinics.

Cash Drops to $7.5 Million as Shares Rise

The New Jersey company has funded its operating losses through a term loan from Avenue Venture Opportunities Fund and by selling stock through an at-the-market offering program. In the second quarter, the quarterly filing included a going-concern warning, even as management said on the call it could reach profitability without a major capital raise.

Cash and marketable securities fell by $2.54 million during the third quarter to $7.50 million, down from $11.61 million at the end of 2025. Total liabilities stood at $19.77 million against $17.26 million in assets, leaving a stockholders' deficit of $2.51 million. Weighted average shares rose 17.9% from a year ago and 5.8% from the second quarter to 9.95 million, showing continued reliance on equity issuance.

What to watch: cash burn and at-the-market share sales in the third-quarter 10-Q filing. Principal payments on the Avenue debt are scheduled to begin in 2027, leaving a narrow cushion if quarterly cash use stays near $2.5 million.

๐Ÿ”ด persistent DISCLOSURE

Risks this quarter didn't answer

Four standing concerns from prior quarters received no new figures or operational updates in the third-quarter earnings release.

  • FDA inspection follow-up: the release's risk factors still warn that the FDA may find the company's response to the recent Form 483 inspectional observations at NeuroMetrix inadequate; the 10-Q filing will show if the agency has replied.
  • Prescription length mix: management did not say what share of VA gammaCore orders were one-year versus three-year prescriptions; call remarks or Q&A will test if the shift to shorter scripts continues.
  • Patent litigation costs: legal spending on the Pulsetto dispute was not broken out in the release; the 10-Q footnote will show the quarterly expense.
  • Chief executive transition: Joshua Lev is now titled Co-Chief Executive Officer alongside President and CFO, but the release did not name the other Co-CEO or explain the governance structure.

Higher Ad Costs Weigh on Consumer Wellness

electroCore sells its non-prescription Truvaga wellness device directly to consumers online. Management said it faced "a more competitive and costly advertising environment" during the third quarter, which prompted its decision to "de-emphasize low-return revenue in our consumer channel". By targeting only higher-converting audiences, the company improved its return on advertising spend by 8% from a year ago, at the cost of a 38.1% drop in channel sales.

๐Ÿ’ฒ Other KPIs

U.S. Prescription revenue (26Q3) $7.95 million
โ‡˜ decelerating

U.S. prescription sales โ€” gammaCore and Quell devices sold mostly to VA hospitals โ€” rose 25.3% from a year ago and 6.8% from the second quarter. Growth slowed from 48.3% in the first quarter and 30.8% in the second, but prescriptions now make up 82.1% of total company revenue.

Cumulative VA gammaCore patients (26Q3) 17,700 patients
โ‡’ stable

The total number of VA patients who have received a gammaCore headache device rose by 1,300 during the quarter, matching the 1,400 added in the second quarter. That brings electroCore's penetration to 3.0% of the estimated addressable VA headache patient pool, up from 2.5% in the first quarter.

TAC-STIM revenue (26Q3) $133 thousand
โ‡„ reversing

Sales of TAC-STIM โ€” a nerve stimulator built for military human performance โ€” fell 27.3% from a year ago and 28.1% from the second quarter. Order timing from defense buyers remains uneven, keeping the product line at 1.4% of total sales.

๐Ÿ”ฎ Guidance

FY26 Revenue $39.0โ€“42.0 million
๐Ÿ ‡ cut from >30% growth (~$41.6M+)
โ‡˜ decelerating

Cut. The new range equates to 26.4% growth at the middle of the range by our math, down from the prior floor of more than 30% growth ($41.6 million). By our math, hitting the $40.5 million middle requires $11.8 million in fourth-quarter sales, a 21.5% jump from the third quarter and 27.4% growth from a year ago.

FY27 Revenue growth 30โ€“40%
โ‡— accelerating

New. The preliminary plan calls for 35% growth at the middle of the range over full-year 2026 sales, which by our math points to roughly $54.7 million in 2027 revenue. Management tied the forecast to expanding VA sales reps, smaller sales territories, and a potential oncology nerve-pain approval for Quell.

Positive Adjusted EBITDA timeline By the end of 2027
๐Ÿ ‡ cut from 2027Q3
โ‡˜ decelerating

Cut. Management now expects its first quarter of positive operating profit excluding one-offs by the end of 2027, slipping from last quarter's target of the third quarter of 2027. It is the fourth time in two years the breakeven date has moved back, though the third-quarter loss narrowed to $0.68 million.

โ“ Key Questions

Who is the other Co-CEO alongside Joshua Lev?

The release titles Joshua Lev as Co-Chief Executive Officer, President, and CFO, after two quarters as Interim President. Management did not name the second Co-CEO or explain how responsibilities are divided.

Why did Quell sales drop from the second quarter?

Quell revenue fell 7.7% from the prior quarter to $1.2 million after growing 30% in the second quarter. Investors need to know if VA clinic onboarding slowed or if distributors adjusted inventory during the Lovell transition.

How much stock was sold under the ATM program?

Weighted average shares rose by 550,000 from the second quarter while cash still dropped by $2.54 million to $7.50 million. The 10-Q needs to show how much cash came from equity sales versus operations.

Has the FDA responded to the NeuroMetrix Form 483?

The company submitted a 510(k) for Quell in chemotherapy nerve pain in October, but risk factors still flag unresolved Form 483 inspection observations on Quell's complaint handling from June.
