Centene (CNC) Q3 2026 earnings review

Centene raises its profit outlook, but its member base shrinks

Centene, a health insurer for government Medicaid, Medicare and Marketplace plans, repaired its profit margins while its customer base shrank. Core revenue barely grew in the third quarter because total membership fell by 8% from a year ago after the company raised prices. Even so, lower overhead after nearly 6,000 job cuts and steadier medical costs in Medicare lifted profit per share, and chief executive Sarah M. London raised the full-year profit outlook.

At a glance
Core premium and service revenue$45.1B +0.5% from a year ago
Health benefits ratio (medical costs)92.7% unchanged from a year ago
Profit per share, excluding one-offs$0.58 +16% from a year ago
Full-year profit per share outlookRaised $0.40 floor now above $5.20

โš–๏ธ Verdict: ๐ŸŸข Bullish

The story got better because Medicare and Marketplace medical costs stayed in check without the loss-reserve help Centene needed last year, and the company cut another $1.1 billion of debt. One caution: a lower tax rate in the quarter did part of the work in the earnings beat, and by our math the unchanged revenue plan points to core sales falling about 9% in the fourth quarter.

The question now is whether higher profit per member can offset a shrinking customer base as state contract exits arrive in 2027. Either Medicare drug plan growth and lower overhead carry earnings forward, or Medicaid contract losses and new federal eligibility rules pull total profit down. Management's first 2027 outlook will settle it.

๐Ÿ‚ Bull Case

๐ŸŸข๐ŸŸข strengthening MARGIN

Higher Prices Fixed Commercial and Medicare Margins

Centene provides government-sponsored health insurance through Medicaid, Medicare and the Affordable Care Act Marketplace. After medical costs jumped last year, the health insurer raised its 2026 Marketplace prices and accepted a smaller customer base to restore profitability. In the third quarter, its consolidated health benefits ratio (HBR), the share of premiums spent on medical care, held steady at 92.7%, matching the best end of what the prior full-year plan implied.

  • Commercial HBR: 89.0%, improved by 0.9 percentage points from a year ago (80.9% over nine months vs 85.3%)
  • Medicare HBR: 93.8%, improved by 0.5 percentage points from a year ago
  • Medicare Prescription Drug Plan (PDP) members: 8.95 million, up 144,200 from last quarter

The company credited "strong fundamental performance in both the Medicare Advantage and PDP businesses" alongside better Marketplace pricing. Medicare margins improved even though Centene had no loss-reserve release to help the segment this year, unlike in 2025. The insurer also continues moving dual-eligible members into integrated Medicare plans.

What to watch: fourth-quarter medical costs. By our math, the updated full-year plan points to a fourth-quarter HBR of about 93.5%, which will test whether winter medical utilization stays within the company's cushion.

๐ŸŸข strengthening MARGIN

Nearly Six Thousand Job Cuts Lowered Overhead

To match its smaller member base, Centene committed to a second voluntary buyout program in July under its enterprise optimization plan. In the third quarter, the St. Louis group recorded $276 million in severance and $35 million in third-party vendor costs, bringing its employee count down by 5,900 people, or 9.9%, in three months.

Those buyout charges pushed the reported overhead ratio (selling, general and administrative expenses as a share of core revenue) up by 0.2 percentage points to 7.2%. Excluding one-offs, however, the overhead ratio fell by 0.5 percentage points from a year ago to 6.5%, helped by cost cuts and growth in Medicare drug plans, which cost less to run.

What to watch: fourth-quarter overhead expenses. The full-year outlook leaves about $90 million in remaining severance and vendor charges for the fourth quarter, and next year's plan will show how much of the payroll savings Centene keeps.

๐ŸŸข strengthening CAPITAL ALLOCATION

Centene Retired Over One Billion in Debt

After a $6.7 billion goodwill write-down pushed its debt ratio up last year, Centene paused stock buybacks and directed its cash toward paying down debt. The company bought back $558 million of its senior notes during the third quarter and retired the remaining $569 million of its 2027 notes in October.

In total, the healthcare enterprise cut its debt by $1.1 billion since June, bringing quarter-end debt to $15.5 billion and lowering its debt-to-capitalization ratio to 41.1%, down from 46.5% at the end of 2025. That allowed Centene to trim its full-year interest expense forecast by $25 million at the middle of the range, to $600โ€“620 million.

What to watch: the remaining $1.9 billion debt repurchase authorization. With the 2027 notes gone and parent-company cash down to $175 million at quarter end, the pace of future debt paydowns depends on subsidiary dividends in the fourth quarter.

๐Ÿ”ด๐Ÿ”ด persistent GROWTH

Shrinking Member Rolls Point to Lower Revenue

While price increases repaired margins, Centene's customer base continues to shrink. Total at-risk membership fell by 2.27 million people from a year ago, or 8.1%, to 25.7 million, and dropped by 187,200 from the second quarter. Because the insurer covers far fewer people, core premium and service revenue grew just 0.5% from a year ago to $45.1 billion.

  • Marketplace members: 3.40 million, down 41.6% from a year ago and down 91,400 from last quarter
  • Medicaid members: 11.90 million, down 6.4% from a year ago and down 214,800 from last quarter
  • Medicaid HBR: 93.6%, improved from 93.9% last quarter, but 0.2 percentage points higher than a year ago

The top-line rollover is now arriving. Centene kept its full-year premium and service revenue plan unchanged at $173โ€“177 billion. By our math, that outlook leaves fourth-quarter core revenue down 8.6% from a year ago at the middle of the range, as the smaller Marketplace and Medicaid books lap last year's larger enrollment.

What to watch: the 2027 financial outlook. Next year brings scheduled contract exits in Florida, Hawaii, California and New York alongside federal Medicaid work requirements, which will test whether Medicare drug plan growth can offset Medicaid losses.

Tax Adjustments and Reserve Releases Aided Profit

"Solid underlying fundamentals within our core businesses have powered a faster recovery to our earnings trajectory than previously anticipated," chief executive Sarah M. London said, as Centene raised its full-year profit floor by $0.40 to greater than $5.20 per share, excluding one-offs. Third-quarter profit per share excluding one-offs rose 16% from a year ago to $0.58.

Two accounting items did much of the work in the quarter. A "decrease to the estimated annual effective tax rate applied to year-to-date operating results" lowered the third-quarter tax rate on adjusted earnings to 14.5%, adding about $0.08 per share by our math against the 26% full-year rate. Across the first nine months, Centene also lowered its estimate of prior-year medical claims by $1.56 billion.

Furthermore, the raised full-year floor of $5.20 still sits $1.25 below the $6.45 per share Centene has already earned in the first nine months. What to watch: fourth-quarter profit per share excluding one-offs, which will show how much of the nine-month gain holds up during the seasonally higher-cost winter quarter.

Risks this quarter didn't answer

Three standing risks from earlier quarters received no new numbers in the third-quarter earnings release.

  • 2027 state contract exits and federal rules: the revenue loss from exiting contracts in California, Hawaii, Florida and New York, plus new federal Medicaid work requirements under the One Big Beautiful Bill Act, awaits 2027 outlook numbers.
  • Marketplace risk-adjustment true-ups: the release cited 2026 risk-transfer revenue as a driver, but did not state the dollar amount; the quarterly filing's footnotes will show the exact accrual.
  • 2025 outlook lawsuits: the status of the federal securities class action and derivative lawsuits over last year's withdrawn forecast awaits the legal contingencies footnote in the quarterly filing.

๐Ÿ’ฒ Other KPIs

Operating cash flow (26Q3) -$3,669 million
โ‡„ reversing

Cash flow swung from a $3.59 billion inflow last quarter to a $3.67 billion outflow in the third quarter, bringing nine-month operating cash flow to $4.29 billion, down 7.8% from a year ago. Centene said the outflow was "primarily driven by the timing of pass through, premium and other payments, as expected". Unregulated cash available for general corporate use fell from $715 million in June to $175 million.

Days in claims payable (26Q3) 48 days
โ‡’ stable

Medical claims reserves stood at $21.5 billion, or 48 days of average medical cost, up one day from the second quarter and unchanged from a year ago. Days in claims payable measures how many days of medical bills the insurer holds in reserve, so a stable reading means current-quarter margins did not come from thinning the reserve cushion.

Premium and trade receivables (26Q3) $20,329 million
โ‡— accelerating

Money owed to Centene from government and commercial programs rose 12.3%, or $2.22 billion, from the end of 2025, outpacing total revenue growth of 1.9% in the third quarter. The build in unpaid premiums and government risk-sharing balances absorbed $2.26 billion of operating cash over the first nine months.

๐Ÿ”ฎ Guidance

FY26 Profit per share, excluding one-offs > $5.20
๐Ÿ … raised from > $4.80
โ‡˜ decelerating

Raised. The healthcare company lifted its full-year profit floor by $0.40 per share. It is the third raise this year: +$0.40, +$1.40, +$0.40. Because Centene already earned $6.45 per share in the first nine months, the new floor still allows a fourth-quarter loss of up to $1.25 per share by our math, compared with a $1.19 loss a year ago.

FY26 GAAP diluted EPS > $3.46
๐Ÿ … raised from > $3.11
โ‡˜ decelerating

Raised. The reported profit floor moved up by $0.35 per share, despite higher full-year severance estimates of $0.57 per share after tax. Against nine-month reported profit of $5.15 per share, the floor leaves room for a fourth-quarter loss of up to $1.69 per share by our math.

FY26 Health benefits ratio 90.4โ€“91.1%
๐Ÿ … raised from 90.5โ€“91.3%
โ‡˜ decelerating

Improved (lowered). The insurer reduced the middle of its full-year medical cost ratio range by 0.15 percentage points to 90.75%, following a 0.40-point improvement last quarter. With the nine-month ratio at 89.9%, the outlook points to a fourth-quarter medical cost ratio of about 93.5% by our math, down from 94.3% in the fourth quarter of 2025.

FY26 Total revenues $196.5โ€“200.5 billion
๐Ÿ … raised from $193.5โ€“197.5 billion
โ‡˜ decelerating

Raised. The middle of the range rose by $3.0 billion, to 1.9% growth for the year. Because the core premium and service revenue outlook did not move, the entire $3.0 billion increase comes from pass-through state premium taxes, which add to both revenue and expense without changing profit.

FY26 Premium and service revenues $173.0โ€“177.0 billion
๐Ÿ † unchanged from $173.0โ€“177.0 billion
โ‡˜ decelerating

Unchanged. The plan still calls for core revenue to stay nearly flat (+0.2% at the middle of the range) against 2025. By our math, subtracting nine-month core revenue of $134.1 billion leaves $40.9 billion for the fourth quarter, a drop of 8.6% from a year ago.

FY26 Overhead ratio, excluding one-offs 6.9โ€“7.5%
๐Ÿ † unchanged from 6.9โ€“7.5%
โ‡˜ decelerating

Unchanged. Centene held its full-year administrative cost plan steady, excluding $355โ€“405 million of severance and $85โ€“115 million of vendor costs. Because the nine-month ratio stands at 7.0%, reaching the 7.2% middle of the range implies fourth-quarter overhead rising to about 7.8% of core revenue on a smaller seasonal revenue base.

FY26 Reported SG&A expense ratio 7.2โ€“7.8%
๐Ÿ † unchanged from 7.2โ€“7.8%
โ‡˜ decelerating

Unchanged. The reported administrative cost outlook stays at 7.5% at the middle of the range. With $379 million of the full-year $440โ€“520 million optimization and severance charges already booked in the first nine months, about $90 million remains for the fourth quarter by our math.

FY26 Interest expense $600โ€“620 million
๐Ÿ … raised from $620โ€“650 million
โ‡˜ decelerating

Improved (lowered). Centene cut its annual interest cost forecast by $25 million at the middle of the range after retiring $1.1 billion of senior notes since June. With $466 million spent in the first nine months, the plan leaves $144 million for the fourth quarter at the middle of the range.

FY26 Effective tax rate (GAAP and excluding one-offs) 26.5โ€“27.5% GAAP / 25.5โ€“26.5% Adjusted
๐Ÿ † unchanged from 26.5โ€“27.5% GAAP / 25.5โ€“26.5% Adjusted
โ‡’ stable

Unchanged. Both full-year tax rate ranges held steady despite the third quarter's 14.5% adjusted tax rate, which caught up year-to-date taxes to the full-year estimate.

FY26 Diluted shares outstanding 497.0โ€“500.0 million
๐Ÿ † unchanged from 497.0โ€“500.0 million
โ‡’ stable

Unchanged. Share count expectations remain steady, reflecting zero common stock buybacks during the third quarter as capital went toward retiring debt.

โ“ Key Questions

How much will 2027 contract exits reduce revenue?

Centene faces scheduled Medicaid and state plan exits in Florida, Hawaii, California and New York by early 2027. Management has not yet sized the combined membership and revenue loss from those four departures.

What was the exact 2026 Marketplace risk-adjustment benefit?

The release named 2026 Marketplace risk-transfer revenue as a driver of both revenue and medical margin in the third quarter, but did not give the dollar figure. Investors need the standalone Marketplace margin excluding prior-period adjustments.

How much of Medicare's 20% growth came from reclassifications?

Centene moved its dual-eligible Medicare-Medicaid members out of Medicaid and into Medicare this year. Without the reclassified dollar amount, outsiders cannot separate underlying Medicare growth from the accounting transfer.

How will Centene fund further debt buybacks with $175M in cash?

Unregulated parent-company cash dropped from $715 million in June to $175 million in September, before Centene spent $569 million retiring notes in October. The timing of fourth-quarter subsidiary dividends will govern liquidity.
