The Cigna Group (CI) Q3 2026 earnings review
Cigna raises its profit outlook, but pharmacy benefit earnings drop
Cigna Group's health insurance and specialty drug units are carrying the company while its pharmacy benefit division shrinks. Third-quarter revenue rose 6% to $73.7 billion, while profit per share excluding one-offs edged up 2% as lower pharmacy benefit margins offset 15% profit growth in both medical insurance and specialty drugs. Chief executive Brian Evanko lifted the full-year profit outlook slightly, yet by our math the new floor still points to fourth-quarter profit per share falling about 13%.
| Total revenue | $73.7B +5.6% from a year ago |
|---|---|
| Profit per share, excluding one-offs | $7.98 +1.9% from a year ago |
| Evernorth pre-tax profit | $1,829M -3.9% from a year ago |
| Full-year profit per share outlook | Raised $0.15 at least $30.60 (+2.5% from a year ago) |
⚖️ Verdict: ⚪ Neutral
The story is unchanged because both halves of Cigna's split business stayed on their existing tracks. Health insurance profits beat what the company's own second-quarter plan pointed to, allowing a $50 million segment outlook raise, while copycat specialty drugs kept lifting Evernorth's specialty arm. One caution: Pharmacy Benefit Services profit dropped 22% as big client renewals cut its margin, and losing another 545,000 pharmacy members since June shows the core pharmacy reset has not stabilized.
The question now is whether specialty drug profit growth comes from new hospital and patient demand or mainly from drug rebates shifting out of the pharmacy benefit unit. If it is new demand, Evernorth's combined profit will grow again once the repriced pharmacy contracts pass their one-year mark; if it is an internal shift, combined pharmacy earnings will stay flat. First-quarter 2027 Evernorth earnings, when the repriced contracts lap, will settle it.
🐂 Bull Case
Health Insurance Rate Increases Carry the Company
Cigna Healthcare, the company's medical insurance division, is carrying all of the group's profit growth this year. Pre-tax operating profit excluding one-offs rose 15% from a year ago to $1,193 million, while revenue climbed 9% to $11,708 million. That result beat the roughly $1,056 million floor implied by our math from the company's second-quarter plan. The insurer cited "premium rate increases to cover expected increases in medical costs" along with higher investment income.
Total medical customers rose 2% from a year ago to 18.45 million, adding 39,000 members since June as gains in middle-market employers offset losses in big national accounts. Because Q3 beat what the earlier outlook pointed to, management raised the division's full-year profit floor by $50 million to at least $4,600 million. Beating an implied target shows the old plan was cautious, so the raise locks in the third-quarter beat rather than upgrading the fourth quarter.
What to watch: January medical customer enrollment and first-quarter premium growth. Those figures will show whether Cigna kept its employer clients after raising premium rates again for 2027, and how smoothly it exits the individual insurance exchange on January 1.
Cheaper Copycat Drugs Lift Specialty Profit 15%
Specialty and Care Services, the Evernorth division that distributes high-cost drugs for complex and rare diseases, grew pre-tax profit excluding one-offs by 15% from a year ago to $1,064 million. That pace cooled from 22% in the second quarter as the unit lapped last year's copycat drug launches, but it still lifted the division to 58% of Evernorth's total profit by our math. Critics argue that part of this profit simply moved over from the regular pharmacy unit.
Revenue in the division rose just 1% to $26,498 million, yet pre-tax margin widened by 0.5 percentage points to 4.0%. The group credited "strong organic growth in specialty businesses, including higher biosimilar and specialty generic adoption" for the profit jump. When patients switch from expensive brand-name biologics to lower-priced copycat versions, Cigna bills less revenue per prescription but keeps a larger dollar profit.
What to watch: fourth-quarter Specialty and Care Services pre-tax profit growth. Holding double-digit growth against a tougher fourth-quarter comparison would show that copycat drug profits are still expanding rather than leveling off.
Positives this quarter didn't test
Two standing growth drivers from earlier quarters received no new numbers in the third-quarter earnings release.
- Hospital and health-system pharmacy services: no breakout for Shields Health Solutions or CarePathRX; annual filing disclosures or call remarks are needed to check provider-administered drug growth.
- Artificial intelligence in care coordination: no update on the Pharmacy Forward program or patient cost savings; future clinical utilization disclosures will test whether these tools lower medical claims.
🐻 Bear Case
Contract Renewals Cut Pharmacy Benefit Profit 22%
Chief executive Brian Evanko said Cigna is "making health care more affordable and personalized, while advancing Accelerate to One to drive productivity, reduce costs, and support sustained long-term earnings growth." Inside Pharmacy Benefit Services, which manages prescription drug plans for employers and insurers, giving clients more affordable pricing is still cutting deeply into Cigna's own earnings. The unit's pre-tax profit excluding one-offs fell 22% from a year ago to $765 million, following 27% drops in both Q1 and Q2.
- Pharmacy Benefit Services revenue: $37,276 million, up 9% from a year ago
- Division pre-tax profit margin: 2.05% by our math, down 0.8 percentage points from 2.86% a year ago
- Total pharmacy customers: 117.7 million, down 4% from a year ago and down 545,000 since June
The company blamed the profit drop on "large client contract renewals and strategic investments" as it builds a new fee-based pharmacy model while keeping major clients on lower-margin contracts. Because the pharmacy drop offset the specialty drug gains, total Evernorth pre-tax profit shrank 4% to $1,829 million.
What to watch: first-quarter 2027 Pharmacy Benefit Services pre-tax profit. Once the repriced multi-year contracts pass their one-year anniversary in January, profit must stop falling to prove the margin reset was a one-time step down.
Employer Claims Push Medical Care Costs Up
Cigna Healthcare's medical care ratio (MCR) — the share of insurance premiums spent on patient medical claims — rose to 85.0% from 84.8% a year ago and 84.5% last quarter. Management attributed the 0.2 percentage point increase to its U.S. employer business mix, which outweighed lower claim costs in the individual family plans Cigna will exit in January. Selling, general and administrative overhead also rose 0.1 percentage points to 4.9% of total revenue.
The insurer narrowed its full-year medical care ratio forecast by 0.5 percentage points to a range of 83.95% to 84.45%, keeping the middle of the range at 84.2%. Because the first two quarters ran lower at 79.8% and 84.5%, holding the 84.2% full-year center after an 85.0% third quarter shows medical cost inflation remains high heading into year-end.
What to watch: fourth-quarter Cigna Healthcare medical care ratio. Landing inside the narrowed full-year band will test whether employer stop-loss claims stay contained during the fourth-quarter deductible season.
The Raised Plan Still Points to a Weak Q4
Cigna raised its full-year profit outlook excluding one-offs by $0.15 to at least $30.60 per share. However, the group has already earned $23.55 per share across the first nine months of 2026. By our math, the new full-year floor leaves at least $7.05 per share for the fourth quarter, which is 13% below the $8.08 per share Cigna earned in the fourth quarter of 2025.
The segment targets tell the same story. Evernorth's unchanged floor of at least $6,900 million leaves $1,942 million for Q4 by our math, down 11% from $2,188 million a year ago. Cigna sets one-sided floors that it usually beats slightly, so a lower implied fourth quarter reflects seasonal caution, but it also shows third-quarter strength did not lift fourth-quarter expectations.
What to watch: fourth-quarter Evernorth pre-tax profit against the implied $1,942 million floor. In each of the past two years, Evernorth finished within $25 million of its annual floor, so the fourth-quarter number will likely match that implied drop.
Risks this quarter didn't answer
Three standing concerns from prior quarters received no new figures in the third-quarter press release.
- Profit shift between pharmacy units: the release again omitted how much of Specialty and Care Services' profit growth came from drug economics shifting out of Pharmacy Benefit Services; conference call remarks or the 10-Q filing are needed.
- Pharmacy benefit regulation: no update on the financial impact of federal delinking rules, state pharmacy laws, or the Federal Trade Commission settlement; future filings will carry any legal or tax adjustments.
- Weight-loss drug demand: no commentary on GLP-1 drug coverage or utilization after employer coverage slipped last quarter; call Q&A will test whether the headwind grew.

Cigna Launches a $3 Billion Cost Plan
At its recent Investor Day, Cigna introduced a multi-year modernization and productivity program called Accelerate to One, targeting $3 billion in savings alongside a long-term goal of 10% to 14% annual growth in profit per share excluding one-offs and about $50 billion of cumulative operating cash flow through 2030. Meanwhile, the company's existing optimization program cost $85 million pre-tax this quarter and $535 million over the first nine months.
💲 Other KPIs
Debt as a share of total capital fell to 41.8% at September 30, down from 43.0% at year-end and 44.9% a year ago. With leverage moving closer to management's 40% target, Cigna doubled its year-to-date share buybacks from $250 million in late July to $500 million by November 4, cutting diluted share count by 1.4% from a year ago.
Behavioral care membership dropped by 4.61 million people from June and 19% from December to 23.01 million. Cigna said the drop came from the "termination of a government contract, with minimal impact to The Cigna Group’s revenues and adjusted income from operations", pulling total customer relationships down 6% year-to-date to 177.6 million.
Net medical costs payable — money Cigna has set aside for medical claims incurred but not yet paid — fell 4.5% from June's $5.09 billion but rose 7.3% from $4.53 billion a year ago, closely tracking the 9% growth in healthcare revenue. Favorable prior-year reserve development reached $356 million over nine months, up from $319 million last year.
🔮 Guidance
Raised. Management lifted its full-year profit floor by $0.15 per share, which points to at least 2.5% growth for the year. By our math, that leaves at least $7.05 per share for the fourth quarter, down 12.8% from $8.08 a year ago. It is the third raise this year following increases of $0.10 in Q1, $0.10 in Q2, and $0.15 in Q3.
Unchanged. The plan still calls for Evernorth pre-tax profit to fall up to 4.4% for the full year against $7,221 million in 2025. By our math, after $4,958 million in the first nine months, the floor leaves $1,942 million for Q4, an 11.2% drop from $2,188 million in Q4 2025. Evernorth finished within $25 million of this floor in both 2024 and 2025.
Raised. The healthcare division's floor moved up $50 million, pointing to at least 10.8% growth for the year. By our math, with $3,983 million already booked, the floor requires only $617 million in Q4, down 15.9% from $734 million a year ago. It is the third straight raise for the unit: +$25M, +$25M, and +$50M.
Unchanged at the middle of the range. The insurer narrowed its full-year medical claim ratio band by 0.5 percentage points around the same 84.2% center, compared with 84.4% for full-year 2025 and 85.0% in the third quarter.
❓ Key Questions
How much specialty profit shifted from the pharmacy benefit unit?
Evernorth's nine-month profit is down 1.5% while Specialty and Care Services rose $574 million and Pharmacy Benefit Services fell $649 million. Without a dollar split, investors cannot tell new specialty demand from an internal accounting shift.
Why did pharmacy customers drop by another 545,000 since June?
Total pharmacy customers have fallen 5% since December to 117.7 million. Management attributed the drop to client transitions and lower health-plan membership, but has not quantified how much revenue leaves with those members in 2027.
How does the $3 billion Accelerate to One plan overlap existing cuts?
Cigna announced a new $3 billion modernization program at Investor Day while its existing optimization program has already racked up $535 million in pre-tax charges this year. Management did not disclose the new program's restructuring charges or net savings timeline.
What is the current medical care ratio inside the stop-loss book?
Employer business mix pushed Cigna Healthcare's medical care ratio up 0.2 percentage points to 85.0%. Because the company stopped disclosing the stop-loss ratio separately, investors cannot verify how much margin recapture remains for 2027.
