Celcuity (CELC) Q3 2026 earnings review
Celcuity wins broad coverage for its cancer drug, while costs climb
Celcuity, a biotechnology company developing targeted cancer medicines, started shipping its first approved breast cancer drug while its launch costs climbed. Because shipments of REVTORPYK began on September 30 and the company books revenue upon delivery, third-quarter revenue was zero while operating cash burn rose 48% to $66 million. In the first five weeks of sales, chief executive Brian Sullivan said 78% of the top 36 cancer centres ordered the medicine and health plans covering 91% of insured Americans agreed to pay for it.
| US health plan coverage for REVTORPYK | 91% of commercial and Medicare lives |
|---|---|
| Top 36 cancer accounts ordering | 78% represents ~50% of eligible patients |
| Operating cash burn | $66.2M +48% from a year ago |
| Cash runway outlook | Unchanged $642.7M funds operations through 2029 |
โ๏ธ Verdict: ๐ข Bullish
The story got better because early insurance coverage, top cancer guideline endorsement and hospital ordering cleared the hardest commercial hurdles in weeks. The company also filed for US approval in the remaining 40% of second-line patients with a PIK3CA mutation and showed stronger prostate cancer responses at its highest dose. One caution: commercial hiring more than tripled overhead expenses, and total cash fell by $111 million after operating losses and a $50 million milestone payment to Pfizer.
The question now is how fast hospital shelf orders turn into paying patients and cash collections. Either clinics infuse patients quickly enough to pull quarterly cash burn below the $49 million average implied by the 2029 runway plan by our math, or slow uptake forces the company to raise more money. Fourth-quarter product revenue and reorder rates will give the first real read.
๐ Bull Case
Broad Early Coverage and Ordering for REVTORPYK
Celcuity launched REVTORPYK, its intravenous drug for advanced breast cancer patients without a PIK3CA gene mutation, after winning US approval in July. In late July, the National Comprehensive Cancer Network gave the medicine "the preferred Category 1 recommendation in the NCCN Guidelines", the highest clinical endorsement for doctors choosing a second-line treatment.
- Insurance coverage: 91% of US commercial and Medicare lives are in health plans covering the drug
- Top clinic adoption: 78% of the top 36 accounts (28 centres by our math, treating half of eligible patients) have placed orders
- Provider pathways: 3 of the 4 largest oncology treatment systems now include the therapy
- Field reach: sales reps have engaged over 3,400 clinical sites that treat 82% of US breast cancer patients
These early access figures matter because insurance barriers and hospital committee delays often slow down new cancer drug launches. Securing coverage for nine in ten insured Americans and inclusion in major clinical pathways within five weeks of launch removes the main administrative hurdles for doctors.
What to watch: fourth-quarter net product revenue and repeat ordering rates across those 36 flagship accounts. High re-order rates will show that clinics are putting multiple patients on therapy rather than just stocking initial pharmacy shelves.
FDA Application Filed to Expand Into Mutant Cancer
REVTORPYK is currently approved only for patients without a PIK3CA mutation, who make up about 60% of the second-line market. During the third quarter, the biotechnology group filed a supplemental application with the US regulator to expand the label to patients who carry that mutation, backed by updated Phase 3 VIKTORIA-1 data presented in October.
In the updated presentation, the three-drug regimen shrank tumours in 59% of evaluable patients and the two-drug regimen shrank them in 43%, compared with 33% for the standard competitor alpelisib. Only 4% to 5% of patients stopped Celcuity's drug due to side effects, versus 19% on alpelisib. Progression-free survival โ the time a patient lives without the cancer worsening โ reached 21.5 months on the three-drug combo versus 9.1 months on control "in patients who had no liver metastases".
An approval would let community oncologists prescribe one therapy regardless of mutation status, while sparing patients the high drop-out rates of older pills. Note that the 21.5-month figure comes from a healthier subgroup without liver spread, whereas the overall mutant trial reported last quarter showed 11.1 to 11.3 months.
What to watch: the US regulator's acceptance of the supplemental filing and the target review date. A priority review schedule would put a broader label on the market in the first half of 2027.
Higher Dose Delivers Stronger Prostate Cancer Responses
Celcuity is testing the same drug, gedatolisib, alongside the hormone pill darolutamide in men with metastatic castration-resistant prostate cancer, a severe form of the disease that keeps growing despite standard hormone-blocking treatment. In October, researchers presented updated Phase 1b/2 trial data across three dose levels: 120 mg, 180 mg and 240 mg.
Blood levels of prostate-specific antigen (PSA), a protein marker of tumour activity, fell by at least half in 67% of patients on the 240 mg dose, up from 47% at 180 mg and 36% at 120 mg. Patients previously treated with abiraterone lived a median of 15.7 months before their scans worsened, compared with 9.1 months for those previously given enzalutamide. No patient quit treatment due to drug-related side effects.
The clear step-up in tumour marker reductions from 120 mg to 240 mg without extra drop-outs gives the Minnesota company a strong reason to push dosing higher. That opens a second large cancer market beyond breast cancer.
What to watch: the design and starting doses of the randomized Phase 2 prostate study, which the company plans to start "in the first half of 2027". That trial will pick the dose for a final Phase 3 registration study.
๐ป Bear Case
September 30 Launch Left Quarter Revenue at Zero
"Our commercial and medical affairs teams have laid the foundation for a strong launch over the past few years, and their efforts are paying off," chief executive Brian Sullivan said. He added that doctors are eager to put patients on therapy following the drug's commercial release.
In the third-quarter financial tables, those efforts brought in zero revenue. Celcuity began shipping REVTORPYK on September 30, the final day of the quarter. The company books sales "when control of REVTORPYK transfers to the healthcare providers or pharmacies, which generally occurs upon delivery", and no packages arrived until October.
Shipping on the last day met the company's goal of a third-quarter launch, so all the early ordering statistics cited in the release happened in October and early November. Moreover, initial orders reflect clinics and specialty distributors putting vials on the shelf, not confirmed patient infusions or net price after insurer rebates.
What to watch: fourth-quarter gross-to-net revenue adjustments and distributor inventory levels in the full-year report. A large gap between shipped vials and actual patient infusions would show that early orders were mostly pharmacy stocking.
Commercial Hiring Tripled Sales and Overhead Costs
Building a national oncology sales force before generating revenue has driven a steep rise in spending. Total operating expenses rose 80.4% from a year ago to $77.3 million, lifting the third-quarter net loss excluding one-offs to $66.8 million, up 79.9% from $37.2 million a year earlier.
- Selling, general and administrative costs: $39.8 million, up 220.3% from $12.4 million a year ago
- Commercial build share: $25.3 million of the $27.4 million overhead increase came from commercial hiring and launch preparation
- Research and development costs: $37.5 million, up 23.3% from $30.4 million on higher staffing and manufacturing
- Quarterly cash outflow: $66.2 million burned in operations, plus a $50.0 million milestone payment to Pfizer
For the first time, sales and administrative overhead exceeded research spending, taking 51.5% of total operating expenses by our math. The drug developer is now carrying a fixed cost base of roughly $310 million a year before its first dollar of sales arrives.
What to watch: quarterly operating cash burn in the fourth quarter and early 2027. If cash burn stays near $66 million a quarter, REVTORPYK sales must ramp fast to meet the company's cash runway target without new borrowing.
๐ Other Themes
Milestone Payment and Debt Pushed Equity Negative
Celcuity paid Pfizer $50.0 million in the third quarter, "triggered by receipt of FDA approval for REVTORPYK" under its licensing deal, and booked the sum as an intangible asset. Combined with $754.0 million of convertible notes on the balance sheet and cumulative operating losses, stockholders' equity flipped from $100.6 million at the end of 2025 to a deficit of $74.0 million. Separately, the company hired veteran executive Byron Robinson as Chief Strategy Officer.
๐ฒ Other KPIs
Total cash and marketable securities fell 14.8% from $754.0 million last quarter, a drop of $111.3 million by our math. After raising convertible debt in June to reach $754.0 million, the drugmaker spent $66.2 million on operations and paid Pfizer $50.0 million upon regulatory approval. The company also booked its first $1.5 million of commercial drug inventory.
The average share count rose 15.9% from 47.6 million a year ago after equity offerings and warrant exercises over the past year. That higher share count held the growth in loss per share to 44.6% ($1.33 vs $0.92), even as GAAP net loss widened by 67.4%. Non-cash stock-based compensation rose 47.8% from a year ago to $10.0 million.
๐ฎ Guidance
Unchanged. Celcuity expects that its $642.7 million in cash and investments "will finance our operations through 2029". By our math, stretching $642.7 million across the 13 quarters from October 2026 through December 2029 allows an average net cash burn of $49.4 million per quarter โ below the $66.2 million burned in the third quarter, so product revenue must cover the gap.
New. Following the third-quarter US supplemental filing for the PIK3CA mutant group, the biotechnology group plans to submit Phase 3 data for both the wild-type and mutant cohorts to regulatory agencies outside the United States "later this year or early next year".
New. After presenting positive dose-escalation data in October, management set a target to begin a randomized Phase 2 trial of gedatolisib plus darolutamide in metastatic prostate cancer "in the first half of 2027" to select the dose for a Phase 3 study.
โ Key Questions
How many patients have actually received an infusion?
Twenty-eight of the top 36 cancer centres have ordered REVTORPYK, but hospital pharmacies often stock initial vials before treating anyone. Investors need to know how many patients have started infusions and whether clinics are placing second orders.
What is the wholesale price and expected insurer discount?
Celcuity says 91% of insured Americans have coverage, yet the release gives no wholesale acquisition cost or gross-to-net discount estimate. Without unit pricing, readers cannot translate prescription numbers into fourth-quarter revenue.
Why highlight the no-liver-metastases subgroup in mutant breast cancer?
The release highlights a 21.5-month progression-free survival in mutant-cohort patients without liver metastases, whereas the overall mutant cohort reported last quarter showed 11.1 to 11.3 months. Did the overall survival or progression curve shift in the full population at the October presentation?
Where does the subcutaneous injection formulation stand?
Management spent much of the first-quarter call discussing an under-the-skin version of gedatolisib to extend patent life, but this release only mentions it in boilerplate disclaimers. Has the company selected a final formulation for human testing?
