BrightSpring Health Services (BTSG) Q3 2026 earnings review
BrightSpring raises its 2026 outlook as prescription profit hits a record
BrightSpring, a provider of specialty pharmacy and home health care services, sped up in the third quarter despite tougher year-ago comparisons. Revenue rose 26%, above what the company's own plan pointed to, and gross profit per prescription reached a record $28.86. Chief executive Jon Rousseau raised the full-year outlook again, while strong cash collection cut debt leverage to 1.79 times.
| Revenue | $4,213M +26.4% from a year ago |
|---|---|
| Pharmacy gross profit per prescription | $28.86 +26.8% from a year ago ($27.50 in Q2) |
| Debt leverage ratio | 1.79x down from 2.15x in Q2 |
| Full-year revenue outlook | Raised $750M middle of the range: 24.0% growth |
⚖️ Verdict: 🟢 Bullish
The story got better because pharmacy profit per prescription hit a new high after dipping last quarter, and third-quarter sales cleared the hardest comparison of the year. That $750 million revenue raise shows the prior second-half outlook was cautious. One caution: pharmacy growth still comes from drug prices and mix, as total prescriptions dispensed barely moved.
The question now is whether pharmacy profit per prescription can keep climbing above $29 or is settling onto a plateau. If specialty drug mix and support fees keep expanding, unit profit rises further; if generic conversions level off, flat prescription volume will cap profit growth. Fourth-quarter gross profit per prescription and the specialty revenue split in the quarterly filing will tell.
🐂 Bull Case
Pharmacy Profit Per Prescription Hit a Record
BrightSpring earns most of its sales in Pharmacy Solutions, which delivers specialty and chronic medications to patients at home and in care facilities. Last quarter, gross profit per prescription dipped from $28.03 to $27.50, raising questions about whether its profit margin had peaked.
In the third quarter, gross profit per prescription rose to a record $28.86, up 26.8% from a year ago and 4.9% from last quarter by our math. That helped lift total gross margin by 0.6 percentage points and operating margin by 1.2 percentage points. Cost of goods and services fell 0.6 percentage points of revenue, and selling, general and administrative overhead dropped 0.6 percentage points.
What to watch: gross profit per prescription in the fourth quarter. Another reading near $29 would show that specialty drug mix is still lifting unit profit rather than flattening out.
Third-Quarter Sales Cleared the Tough Comparison
Coming into the second half of 2026, the main worry was a sharp slowdown against last year's strong second half. The prior full-year outlook pointed to second-half sales growth of 10.6% to 15.3% by our math, down from 24.2% in the first half.
Instead, third-quarter revenue grew 26.4% from a year ago to $4,213 million, speeding up from 23.0% in the second quarter. BrightSpring raised its full-year revenue forecast by $750 million at the middle of the range, all from Pharmacy Solutions. Even after that raise, the new plan implies fourth-quarter revenue growth of 21.5% by our math.
What to watch: fourth-quarter revenue growth against that 21.5% implied rate. Clearing the third quarter eased the second-half slowdown concern, and fourth-quarter results will settle how the company enters 2027.
Cash Flow Doubled and Debt Leverage Dropped
The healthcare group has spent the past year cutting debt after selling its Community Living unit in March. Management had targeted debt leverage — net debt divided by annual operating cash profit — below 2.0 times by year-end, along with about $600 million in operating cash flow for 2026.
Leverage fell to 1.79 times on September 30 from 2.15 times in June, beating the year-end goal a quarter early. Operating cash flow nearly doubled to $204.3 million from $107.9 million a year ago, as a $142.4 million rise in trade accounts payable offset higher inventory and receivables. BrightSpring bought back no shares this quarter after spending $120 million during sponsor stock sales in the first half.
What to watch: fourth-quarter operating cash flow. By our math, the company needs about $229 million in the fourth quarter to reach its $600 million full-year target on a reported basis.
Positives this quarter didn't test
Three standing growth and margin drivers were not readable in the earnings release because their numbers appear in the quarterly filing or on the conference call.
- Specialty and infusion revenue and limited-distribution drugs (LDDs, specialty medications only a few pharmacies may dispense): the filing will show whether specialty growth held above 18% and whether the LDD count rose from 155.
- Fee-for-service and hub programs (patient onboarding and support services for drugmakers): the call will show their contribution to pharmacy profit per prescription.
- Internal automation and artificial intelligence tools: SG&A fell 0.6 percentage points of sales in the release, and the call will show how much came from workflow automation.
🐻 Bear Case
Pharmacy Growth Still Relies on Price and Mix
Chief executive Jon Rousseau said the company is positioned to "deliver durable growth in 2027 and beyond" across its platform. In Pharmacy Solutions, which brings in 89% of total revenue, that growth comes almost entirely from higher-priced drugs rather than more prescriptions.
- Third-quarter prescriptions dispensed: 10.95 million, up 1.5% from a year ago
- Nine-month prescriptions dispensed: 32.52 million, unchanged from 32.52 million a year ago
- Third-quarter revenue per prescription: $340.86, up 24.0% from a year ago
This matters because total pharmacy profit equals prescriptions times profit per prescription. With total prescription volume barely moving, any slowdown in the shift toward expensive specialty medications or generic conversions would quickly slow profit growth.
What to watch: specialty prescription growth versus Home and Community prescription volume in the quarterly filing. Continued volume declines in institutional pharmacies would keep the company dependent on price and mix.
Risks this quarter didn't answer
Five standing business, policy and disclosure concerns got no new numbers in the press release.
- Inflation Reduction Act (IRA) drug repricing in Home and Community Pharmacy: the quarterly filing's sub-segment table will show the third-quarter revenue hit ahead of another repricing round in 2027.
- Acquired Amedisys and LHC home health branch margins: BrightSpring kept its 2026 cash profit target for the branches at about $35 million, but the release does not give their standalone profit margin.
- Pharmacy benefit manager (PBM) steering to insurer-owned specialty pharmacies: unaddressed in the release; any commentary will come on the call.
- Medicare home health rates for 2027 and potential 2028 generic drug tariffs: awaiting final federal rules.
- Value-based care patient counts and shared-savings profit: unmentioned for a fifth straight quarter.

Sub-Segment Realignment and Recurring Profit Adjustments
On July 1, BrightSpring "realigned its Rehab in Motion business from the Rehab Care sub-segment to the Home Health Care sub-segment" and recast prior patient counts. Separately, third-quarter Ebitda excluding one-offs of $227.8 million added back $8.2 million of restructuring and divestiture costs and $2.9 million of acquisition costs, down from $17.7 million combined a year ago by our math.
💲 Other KPIs
Daily patient census in Home Health Care rose 57.6% from a year ago and added 808 patients from the second quarter by our math, after recasting for the Rehab in Motion transfer. Year-over-year growth reflects the 107 Amedisys and LHC branches acquired late last year, which lifted Provider Services revenue 31.0% to $480.8 million.
Money owed by customers and insurers rose 20.1% from a year ago and 6.8% from June by our math, growing slower than the 26.4% rise in quarterly revenue. Even so, unpaid bills absorbed $83.4 million of operating cash in the third quarter and $235.9 million over the first nine months as the pharmacy business expanded.
Weighted average diluted shares rose 1.0% from 218.0 million a year ago and edged down slightly from 220.3 million in the second quarter. The first-half buybacks of 2.49 million shares during KKR secondary offerings offset employee stock awards, while basic shares stood at 207.0 million.
🔮 Guidance
Raised. The middle of the range moved up $750 million to 24.0% growth over 2025 continuing operations. By our math, that leaves $4,312 million or 21.5% growth for the fourth quarter, compared with 26.4% in the third quarter. It is the third raise this year, and the largest yet: +$250M, +$287.5M, +$750M.
Raised. The middle of the range rose $750 million to 23.2% growth, accounting for the entire company revenue raise. By our math, the outlook leaves $3,789 million or 20.0% growth for the fourth quarter, down from 25.8% in the third quarter. The three raises this year have grown each quarter: +$225M, +$275M, +$750M.
Unchanged. The plan still calls for 29.7% to 31.4% growth over 2025. By our math, that implies $523 million or 32.8% growth in the fourth quarter, up from 31.0% in the third quarter. After two small raises earlier in the year (+$25M in Q1, +$12.5M in Q2), management kept this range steady.
Raised. The middle of the range rose $22.5 million to 38.4% growth. By our math, that leaves $232 million or 26.4% growth for the fourth quarter, down from 42.0% in the third quarter. BrightSpring has now raised or beaten its full-year cash profit outlook for seven straight quarters, adding $35M, $22.5M and $22.5M this year.
Unchanged. "The Amedisys and LHC branches acquisition is expected to contribute approximately $35 million in Adjusted EBITDA in 2026," BrightSpring said. Management raised this target from $30 million last quarter and held it steady today, counting on a second-half pickup after $17 million of contribution in the first half.
❓ Key Questions
How fast did Specialty and Infusion grow in Q3?
The earnings release only gives total Pharmacy Solutions revenue (+25.8%). The quarterly filing will show how Specialty and Infusion performed against last year's 42% third-quarter growth, and how much Home and Community Pharmacy shrank under federal drug repricing.
How many limited-distribution drugs were added this quarter?
The portfolio stood at 155 drugs in the second quarter after adding two-to-four per quarter in the first half. Because the count is only given on the call, investors need to see whether new drug wins kept pace.
What is the profit margin on the acquired home health branches?
Provider Services cash profit margin was 16.4% in the third quarter, and the full-year contribution forecast for the Amedisys and LHC branches stayed at $35 million. Management has not yet given a timeline for those branches to match legacy margins.
Did the $600 million operating cash flow target change?
BrightSpring raised its full-year cash profit outlook by $22.5 million and generated $204.3 million of operating cash in the third quarter, bringing the nine-month total to $371.2 million. The release did not update the $600 million full-year cash flow target.
