BD (BDX) Q4 2026 earnings review

BD's newer medical devices grow fast, but next year's plan slows

BD, the maker of hospital syringes, infusion pumps and surgical devices, finished fiscal 2026 with faster sales across its newer medical devices even as its US infusion pump business shrank. Fourth-quarter revenue rose 5.6% in constant currency, beating what the company's own full-year plan pointed to by more than 3 percentage points by our math. Chief executive Tom Polen credited newer platforms such as patient monitoring and biologic drug delivery, while a $66 million tariff refund and lower production costs lifted profit.

At a glance
Q4 revenue growth (constant currency)5.6% above the ~2–3% implied by the full-year outlook
Q4 profit per share, excluding one-offs$4.15 +27% from a year ago
Full-year free cash flow$2.9B +43% from a year ago
FY27 revenue outlook (constant currency)Low single-digit down from 3.9% in FY26

⚖️ Verdict: ⚪ Neutral

The story is unchanged because next year's outlook locks in the slowdown that the fourth quarter beat. While BioPharma Systems and Interventional accelerated, the fiscal 2027 plan still calls for low single-digit sales growth and about 4% growth in profit per share excluding one-offs. One caution: United States sales in the infusion pump unit fell 6% in the fourth quarter, showing that the post-recall replacement wave is now a drag on growth.

The question now is whether fast-growing units like patient monitoring and biologic syringes are large enough to lift total growth above the mid-single digits once the infusion pump drag ends after fiscal 2027. Optimists point to broad acceleration outside pumps this quarter, while sceptics note management's own plan still caps growth at low single digits next year. Long-term targets at the December 10 investor day will set the benchmark.

Newer Medical Devices Accelerated Across Three Segments

"We delivered a strong fourth quarter, with 5.6% revenue growth and broad-based performance led by our key growth platforms," chief executive Tom Polen said. By our math, the third-quarter outlook pointed to only 2% to 3% fourth-quarter growth in constant currency, because BD faced a hard comparison in hospital infusion pumps. Instead, three of the company's four segments accelerated.

  • Advanced Patient Monitoring (critical-care sensors bought from Edwards Lifesciences): $317 million, up 17.3% in constant currency
  • Interventional (surgical mesh, stents and urology catheters): $1,472 million, up 7.6% in constant currency
  • BioPharma Systems (prefillable syringes and drug-delivery pens for pharmaceutical companies): $748 million, up 7.0% in constant currency
  • Specimen Management (blood-collection tubes and needles): $529 million, up 9.4% in constant currency

This acceleration matters because BioPharma Systems lapped last year's drop in vaccine syringe orders and returned to 7.0% constant-currency growth, up from 5.2% in the third quarter. The medical device maker also launched its Liverty liver stent in the United States and deployed its first Vmax 160 pharmacy robot at Fairview Health Services.

What to watch: BioPharma Systems constant-currency growth in the first quarter of fiscal 2027. Holding 7% or better would confirm that biologic drug delivery is driving the segment now that the vaccine drop has passed.

🟢 strengthening MARGIN

Lower Production Costs Lifted Fourth-Quarter Gross Margin

BD's reported gross margin had lagged or stayed near flat earlier in the year as import tariffs raised manufacturing costs. In the fourth quarter, GAAP gross margin rose 3.8 percentage points from a year ago to 51.1%, as the cost of products sold fell from 52.7% of revenue to 48.9%.

Part of that gain came from a one-time $66 million benefit in cost of products sold "related to refunds received of tariffs imposed under the International Emergency Economic Powers Act", BD said. By our math, that refund added 1.3 percentage points to gross margin, so underlying gross margin still expanded by 2.5 percentage points on factory productivity under its BD Excellence programme.

What to watch: adjusted operating margin in the full-year filing or conference call remarks. The press release omitted the adjusted gross and operating margin table, which is needed to verify whether BD hit its 25% full-year target.

🟢 strengthening CAPITAL ALLOCATION

Spin-Off Cash Funded Buybacks and Debt Reduction

Polen noted that BD "completed BD's transformation to a more focused MedTech company with separation of our Life Sciences business" in February 2026. The transaction brought in a $3.92 billion net cash distribution from the spin-off entity, which management split between shrinking the share count and paying down debt.

Over the full year, the New Jersey group spent $2.43 billion on share repurchases and made $2.70 billion of debt payments. By our math, fourth-quarter diluted shares fell 4.6% from a year ago to 274.1 million, while net debt dropped 11.3% to $15.33 billion. Because net Leverage sat at 2.9 times EBITDA in the third quarter against a 2.5 times target, investors debated whether buybacks were moving ahead of balance-sheet repair.

What to watch: the year-end net leverage ratio at the December 10 investor day. A drop toward the 2.5 times target would show that fourth-quarter cash generation reduced leverage alongside the buybacks.

🐻 Bear Case

🔴🔴 persistent GROWTH contradicts narrative

Infusion Pump Comparisons Cap Next Year's Plan

Polen said the medical technology group enters fiscal 2027 "from a position of strength, with a portfolio built for durable performance". Yet the company's first outlook for fiscal 2027 calls for constant-currency revenue growth to slow to the low single digits, down from 3.9% in fiscal 2026 and 5.6% in the fourth quarter.

The slowdown comes from Medication Management Solutions, the unit that sells Alaris hospital infusion pumps and pharmacy automation. In the fourth quarter, United States sales in that unit fell 5.6% to $736 million as BD lapped last year's record wave of pump replacements following FDA clearance. That drag pulled total Connected Care constant-currency growth down to 2.1%.

Because the pump replacement wave peaked across fiscal 2025 and 2026, lower capital equipment installations will weigh on revenue and profit margins throughout fiscal 2027. That is why the company's adjusted diluted earnings per share plan of $13.10 to $13.30 represents only 3.1% to 4.7% growth, down from 6.7% in fiscal 2026.

What to watch: United States Medication Management Solutions revenue in the first quarter of fiscal 2027. A drop worse than 5.6% would show the pump comparison drag is deepening as the replacement cycle finishes.

🔴 persistent MARGIN

One-Off Charges Took Half of Operating Profit

BD reported $12.70 of full-year diluted earnings per share excluding one-offs, but GAAP diluted earnings per share from continuing operations were $5.06. While fourth-quarter GAAP operating margin jumped 8.9 percentage points because legal charges fell by 3.2 points of revenue, full-year charges outside ordinary operations remained heavy.

  • Purchase accounting adjustments (mostly non-cash amortisation from past acquisitions): $1.46 billion pre-tax, or $5.21 per share
  • Restructuring and integration costs: $807 million pre-tax, or $2.88 per share, including $450 million of asset write-downs
  • Product remediation, litigation and other items: $204 million pre-tax, or $0.73 per share, including $120 million for legal matters and $68 million for product fixes

Management wrote down $450 million of property and intangible assets to "exit certain operational activities and projects which no longer align with and facilitate its current operational strategy, Excellence Unleashed". Because restructuring, remediation and legal charges have appeared in every quarter over the past two years, they continue to drain cash and GAAP profit.

What to watch: full-year litigation accrual balances and hernia or port trial updates in the Form 10-K filing. Rising claims or accruals above the prior $1.6 billion reserve would mean cash legal outflows will persist.

🔴 persistent DISCLOSURE

Risks this quarter didn't answer

Four standing concerns and watch items cannot be read from the fourth-quarter earnings release and await the conference call or the annual Form 10-K filing.

  • El Paso FDA warning letter and ChloraPrep recalls: the release notes $68 million of full-year product remediation charges but gives no update on recall scope or export certificates, which the 10-K regulatory note will cover.
  • GLP-1 drug-delivery agreements and revenue: the release does not update the ~100 contracted programmes from the third quarter or give dollar sales, which management typically addresses on the call.
  • China volume-based procurement and emerging markets: total international revenue grew 7.6% in constant currency, but the China and emerging-market breakdown awaits the 10-K.
  • Tariff refund receivable collectability: the release records $66 million of tariff refunds received, while the remaining balance-sheet receivable and oil-cost assumptions for fiscal 2027 await the 10-K.

US Manufacturing Deal Targets Future Tariff Relief

BD announced "a landmark partnership with the U.S. Government to expand domestic manufacturing and strengthen the resilience of supply chains for America's essential medical consumables". The company plans to spend $19 billion in the United States over several years, including $3 billion for factory expansion, in exchange for "relief from future tariffs imposed under Section 232 on covered BD products and inputs" as it hits agreed milestones.

💲 Other KPIs

Free cash flow (FY26) $2,933 million
⇗ accelerating

Full-year free cash flow — operating cash from continuing operations minus capital spending — jumped 42.5% from a year ago to $2.93 billion, as smaller working-capital outflows lifted operating cash flow by 30.0% and capital spending dipped 6.0%. By our math, fourth-quarter free cash flow was $1.21 billion, up 39.6% from a year ago and equal to 22.8% of revenue.

Trade receivables, net (26Q4) $2,771 million
⇗ accelerating

Money owed by customers for delivered products rose 15.7% from a year ago to $2.77 billion at year-end, growing three times as fast as fourth-quarter revenue (up 5.4%). In the third quarter, trade receivables were down 1.3% from the prior September, so the fourth-quarter sales beat came with a late-quarter build in uncollected billings.

🔮 Guidance

FY27 Revenue growth (constant currency) Low single-digit
🠆 unchanged from Low single-digit (qualitative framework)
⇘ decelerating

Unchanged. Management formalised its fiscal 2027 outlook at low single-digit constant-currency revenue growth, down from 3.9% in fiscal 2026 and 5.6% in the fourth quarter. The slowdown reflects the full-year comparison drag from record prior-year Alaris infusion pump installations, which management previously estimated at about 2 percentage points of growth.

FY27 GAAP revenue growth Low single-digit
⇘ decelerating

New. Reported revenue is planned to grow in the low single digits, slowing from 4.9% in fiscal 2026. Unlike fiscal 2026, when foreign exchange added 1.0 percentage point to reported sales, the fiscal 2027 plan includes an illustrative currency headwind of about 0.7 percentage points.

FY27 Adjusted diluted EPS $13.10–$13.30
⇘ decelerating

New. The plan calls for profit per share excluding one-offs to rise 3.1% to 4.7% (about 3.9% at the middle of the range), slowing from 6.7% growth in fiscal 2026. Over the last two years, BD finished 0.2% above its final full-year midpoint on average; if the pattern holds, full-year profit would land near the top of the initial range.

❓ Key Questions

What was the adjusted operating margin in Q4 and FY26?

BD guided to an adjusted operating margin of about 25.0% for fiscal 2026, which required roughly 29% in the fourth quarter by our math, but the release omitted the non-GAAP margin reconciliation table.

How much of the $19B US commitment is new spending?

BD pledged $19 billion of US capital, operational and supply-chain investments (including $3 billion for manufacturing) for Section 232 tariff relief, while annual capital spending is currently only $672 million.

What is the status of the El Paso FDA warning letter?

Full-year product remediation charges reached $68 million ($26 million in the second half), and the release gave no update on ChloraPrep recalls or export certificates at the El Paso facility.

How much revenue did GLP-1 drug delivery generate in FY26?

BioPharma Systems accelerated to 7.0% constant-currency growth in the fourth quarter, but BD has not updated dollar progress toward its $1 billion end-of-decade GLP-1 target since the first quarter.
