Axsome Therapeutics (AXSM) Q3 2026 earnings review

Axsome's depression pill sales speed up, and its losses narrow

Axsome Therapeutics, a maker of prescription medicines for brain disorders, grew sales quickly while holding its marketing costs steady. Total revenue rose 52% from a year ago to $260 million, or 47% by our math excluding an $8.5 million milestone payment on its sleepiness drug Sunosi. Chief executive Herriot Tabuteau said demand for its depression and Alzheimer's agitation pill Auvelity drove the quarter, and the net loss narrowed to $13 million because sales force spending stopped climbing.

⚖️ Verdict: 🟢 Bullish

The story got better because Auvelity prescription growth sped up to 40% while sales and overhead expenses dropped slightly from last quarter, proving the company can grow into its doubled sales team. One caution: the company still withholds how many prescriptions come from Alzheimer's agitation, pushed its binge eating trial results into early 2027, and kept $70 million of short-term debt on its balance sheet.

The question now is whether the surge in Auvelity prescriptions among older patients marks the start of a multi-year Alzheimer's ramp or mostly reflects initial trial and the one-time doubling of sales representatives. If nursing homes and specialists adopt the drug broadly, prescription growth will stay near 40%; if the sales force expansion drove a one-off bump, growth will slow once that hiring laps next spring. Next quarter's older-patient prescription numbers and the filing's cash flow statement will tell.

🐂 Bull Case

Auvelity Prescriptions Speed Up After New Approval

Auvelity, Axsome's pill for major depression and agitation in Alzheimer's disease, drives four-fifths of company revenue. Chief executive Herriot Tabuteau said momentum came from "strong underlying demand for Auvelity in major depressive disorder, and continued acceleration of new patient initiations in Alzheimer’s disease agitation." After doubling its sales team and launching the Alzheimer's use in June, total prescriptions grew 40% from a year ago, ending four quarters of slowing growth.

  • Auvelity net sales: $207.6 million, up 53% from a year ago and 15% from last quarter
  • Total Auvelity prescriptions: about 302,000, up 14% from last quarter, adding 36,000 prescriptions by our math
  • New patient starts among people aged 65 and older: up 211% from a year ago and 99% from last quarter
  • Total prescriptions among people aged 65 and older: up 101% from a year ago and 43% from last quarter

Because older patients make up the Alzheimer's group, the doubling of new starts in that age bracket shows doctors are adopting the drug for its second use. Net revenue per prescription held near $687 by our math, because 100% government insurance coverage supported pricing.

What to watch: fourth-quarter prescription growth in patients aged 65 and older. Another fast gain would show that nursing homes and specialists are ordering refills rather than just trying initial samples.

🟢🟢 strengthening MARGIN

Sales Costs Plateau and Operating Losses Narrow

Axsome spent heavily over the past year to hire about 330 extra sales representatives and launch Auvelity for Alzheimer's agitation. Last quarter, management said quarterly selling, general and administrative expenses — the cost of the sales force, advertising and overhead — would plateau near $208.1 million. In the third quarter, those costs fell to $203.1 million, beating the plan by 2.4% by our math.

Because revenue rose $41.3 million from last quarter while sales and overhead spending dropped $5.0 million, the operating loss shrank from $49.6 million in the second quarter to $11.1 million. Compared with a year ago, operating margin improved by 22.7 percentage points to minus 4.3% by our math, as sales and overhead fell by 9.7 percentage points of revenue and contingent payment revaluation swung by 8.6 points.

What to watch: fourth-quarter selling, general and administrative spending. Staying near $203 million while revenue grows would push the drugmaker into operating profit.

Smoking Trial Starts as Pipeline Nears Decisions

Axsome wants to expand its brain-disorder portfolio by testing existing drugs in new illnesses and developing new compounds across ten conditions. In August, the company started its SUMMIT late-stage trial testing AXS-05, the active medicine in Auvelity, as a treatment to help people quit smoking, after delaying the start from earlier this year.

The drugmaker now runs seven late-stage trials and awaits a May 1, 2027 regulatory decision on AXS-12, its experimental pill for sudden muscle weakness in narcolepsy. It also finished enrolling patients in the ENGAGE trial of solriamfetol, the active medicine in its sleepiness drug Sunosi, for binge eating disorder. What to watch: topline results from five late-stage trials in 2027. Positive readouts in adult binge eating and childhood attention-deficit disorder would open large new markets for solriamfetol.

Binge Eating Trial Slips to Next Year

Clinical trial delays have been a recurring issue across Axsome's research programs over the past two years. Three months ago, management planned to report results from the ENGAGE late-stage trial of solriamfetol in binge eating disorder during the fourth quarter of 2026.

In the third-quarter release, the company pushed that readout into the first quarter of 2027, even though the study reached its target enrollment of 450 patients. That follows earlier shifts on the smoking cessation trial, which finally began in August after targets moved from late 2025 to the second quarter of 2026, and the shift work disorder study, which moved from 2026 into the second half of 2027.

What to watch: first-quarter 2027 topline results from the ENGAGE binge eating trial. Any further delay after full enrollment would raise questions about trial execution in a 12-week treatment study.

🔴🔴 persistent CASH contradicts narrative

Milestone Payment Lifts Revenue While Borrowing Persists

Tabuteau highlighted "surpassing a one-billion-dollar annual run rate" as total revenue grew 52% to $259.7 million, and the company said its $313.9 million in cash will fund operations into positive cash flow. However, a one-time $8.5 million milestone payment on Sunosi, its daytime sleepiness medicine, lifted the quarter's revenue and profit.

  • Total revenue excluding the $8.5 million milestone: $251.2 million, up 46.9% from a year ago by our math
  • Sunosi underlying product sales: $38.5 million, up 21.8% by our math, versus 47% headline Sunosi growth
  • Short-term borrowings at quarter-end: $70.0 million, unchanged from December and June
  • Net cash after $197.3 million in total debt and finance leases: $116.6 million by our math

Without the $8.5 million milestone and a $2.4 million non-cash accounting gain on contingent payments, the operating loss would have been $22.0 million instead of $11.1 million by our math. Meanwhile, cash fell by $6.0 million from June while receivables jumped by $65.7 million, and the company again held a $70.0 million short-term credit line draw on its balance sheet at quarter-end.

What to watch: the cash flow statement and debt footnotes in the quarterly filing. They will show whether Axsome burned cash in operations and whether it repaid the $70.0 million credit line right after the quarter closed.

🔴 persistent DISCLOSURE

Long-Term Sales Targets Still Lack a Timeline

In its investor presentation, Axsome repeats its claim that Auvelity can reach more than $8 billion in annual peak sales and that its full portfolio can exceed $18 billion. At the same time, management provides no annual or quarterly revenue forecast.

Without a target year for the $8 billion goal or a near-term sales plan, investors cannot test whether Auvelity's current $830 million annualized pace is on track for management's ambition. What to watch: whether management introduces a full-year 2027 revenue forecast on the fourth-quarter earnings call. Refusing to guide once annual sales pass $1 billion would leave long-term targets unverifiable.

🔴 persistent DISCLOSURE

Risks this quarter didn't answer

Four standing concerns from prior quarters received no new data in the earnings release or presentation because the conference call and quarterly filing were not included.

  • Alzheimer's agitation share of Auvelity: management gave prescription growth for patients 65 and older, but still withheld the exact share of prescriptions written for Alzheimer's agitation versus depression and the share of free samples.
  • Orexin-2 competition in narcolepsy: rival drugs targeting orexin receptors could limit the market for AXS-12 and Sunosi; any updated defense awaits the conference call.
  • Medicare Part D discount rules and price negotiation: a growing share of older patients exposes Auvelity to mandatory government discounts now and potential price negotiation around 2029 to 2031.
  • Related-party royalty to the chief executive's entity: payments scale up alongside Auvelity sales; the quarterly filing's footnote will show the third-quarter expense.

What to watch: the related-party and revenue footnotes in the upcoming third-quarter filing. They will disclose the exact royalty paid and any changes in rebate reserves.

❓ Key Questions

What share of Auvelity prescriptions comes from Alzheimer's agitation?

Prescriptions for patients 65 and older doubled from a year ago, but older adults also take Auvelity for depression. Without the indication split or the volume of free samples, investors cannot measure the true commercial uptake of the June Alzheimer's launch.

Why did Axsome keep $70 million of short-term debt?

Short-term borrowings stood at $70.0 million at the end of December, March, June and September. In the first half, the company repaid the credit line after quarter-end, raising the question of whether it borrows to meet quarterly liquidity rules.

What pushed binge eating trial results into 2027?

Axsome finished enrolling the 12-week ENGAGE study of solriamfetol in binge eating disorder, yet pushed topline results from the fourth quarter of 2026 to the first quarter of 2027. Management did not explain what caused the delay.

Why did accounts receivable jump $65.7 million this quarter?

Receivables rose 23.7% from last quarter while total revenue grew 18.9%. Investors need to know whether wholesalers stocked extra inventory late in the quarter or whether payment terms lengthened for the new Alzheimer's patient base.
