Axogen (AXGN) Q3 2026 earnings review

Axogen's nerve-repair sales speed up, while stock offerings dilute shares

Axogen, the maker of surgical grafts for repairing severed peripheral nerves, grew faster while turning revenue gains back into profit. Third-quarter sales rose 26%, or nearly 30% by our math after stripping out a one-time order pull-forward a year ago, and gross margin bounced back to 74.8%. Chief executive Michael Dale raised the full-year outlook for sales from businesses owned a year ago to at least 26% growth and added $7 million from the newly closed BioCircuit purchase.

At a glance
Revenue$75.8M +26.2% from a year ago (+29.7% ex last year's pull-forward)
Gross margin74.8% up from 72.7% last quarter; down from 76.6% a year ago
US commercial insurance coverage96% up from 86% last quarter; hit 2028 goal early
Full-year underlying revenue outlookRaised $4M at least $283M (+26%), plus $7M from BioCircuit

โš–๏ธ Verdict: ๐ŸŸข Bullish

The story got better because third-quarter results reversed last quarter's profit squeeze and cleared three major milestones early. Ebitda margin excluding one-offs expanded more than 2 percentage points to 17.7%, Aetna and Elevance lifted commercial insurance coverage to 96% two years ahead of plan, and management greenlit a 2027 launch in prostate surgery. One caution: the company sold 5.6 million new shares to buy BioCircuit for $200 million, lifting the diluted share count by 25% from a year ago.

The question now is whether the rebound in unit profitability will hold as breast reconstruction and the new NerveTape device grow. Optimists see higher hospital reimbursement and rising account productivity lifting margins, while skeptics point out that the unchanged 73% full-year gross margin floor allows fourth-quarter margin to drop to 70% by our math. Fourth-quarter gross margin and the 2027 financial plan due in the first quarter will settle it.

๐ŸŸข๐ŸŸข strengthening GROWTH

Sales Acceleration Restored Operating Leverage

Axogen sells human nerve grafts and surgical wraps that help surgeons repair damaged peripheral nerves. Last quarter, rising expenses ate up its sales growth and pushed operating profit down. Chief executive Michael Dale said this quarter's results "validate our market development strategies and the strength of our commercial execution." The numbers back him up because faster revenue growth outpaced costs and lifted profit margins.

  • Revenue: $75.8 million, up 26.2% from a year ago, or 29.7% by our math excluding a $1.6 million order pull-forward in last year's third quarter
  • Breast reconstruction nerve repair: grew more than 50% from a year ago
  • High-potential hospital accounts: revenue per account rose 22% year-to-date, ahead of the 18% annual goal
  • Sales and marketing expense: fell to 40.0% of revenue from 42.7% a year ago
  • Ebitda excluding one-offs: 17.7% of revenue, up 2.3 percentage points from a year ago

The medical-device maker also raised its full-year outlook for sales from businesses it owned a year ago by $4 million to at least $283 million. By our math, that floor points to at least $76.0 million in the fourth quarter, or 26.8% growth, ending the standing concern that growth would slow in the second half.

What to watch: fourth-quarter sales from businesses owned a year ago against that $76.0 million floor. A result above it with Ebitda margin excluding one-offs holding near 17.7% would confirm that operating leverage has turned positive for good.

Insurance Coverage Reached 96% Two Years Early

Avance is Axogen's processed human nerve tissue for bridging severed nerves. Health insurers used to label the graft experimental, which forced surgeons to fight for payment and slowed hospital adoption. After the Food and Drug Administration approved Avance as a biologic drug late last year, major insurers began updating their policies, though coverage had paused at 86% of US commercial plans in the second quarter.

In the third quarter, positive coverage decisions from Elevance and Aetna lifted Avance commercial coverage by 10 percentage points to about 96% of covered Americans. The nerve-repair group noted that it "Achieved near-universal US coverage milestone ahead of plan", reaching a goal originally set for the second half of 2028. Since mid-2025, 15 insurers have added 87.6 million covered people.

This matters because management previously noted that hospitals and surgeons need at least six months after an insurer changes its policy to update local billing contracts. Because Cigna, Elevance, and Aetna all joined within the past two quarters, most of the revenue from those 87.6 million newly covered patientsstill lies ahead in 2027.

What to watch: the 2027 revenue outlook and updated multi-year targets promised for the first quarter of 2027. Raising the multi-year growth target above the current 15% to 20% annual range would show that near-universal insurance coverage is turning into faster hospital ordering.

Sales Hiring Beat Plan and Prostate Moves Ahead

Axogen grows by training surgeons and adding specialized sales representatives in four surgical markets: limb trauma, breast reconstruction, jaw and neck surgery, and robotic prostate cancer surgery. For two years, prostate nerve repair remained a small pilot study while investors waited to see if the company would commit to a commercial launch.

This quarter, the Florida-based company passed its full-year sales hiring targets ahead of schedule, reaching 31 breast reps and 132 extremities reps against year-end goals of about 30 and 130. It also decided to "initiate Prostate market development beginning in 2027" after completing more than 220 procedures across 15 clinical sites, and it enrolled the first patient in Nerve-RESTORE, its clinical study for motor nerves.

What to watch: the prostate investor presentation on November 9, 2026, where management and urologic surgeons will share the clinical study results. Strong recovery rates for patient continence and potency would validate a prostate market the presentation estimates at $754 million a year.

๐Ÿป Bear Case

๐Ÿ”ด persistent MARGIN contradicts narrative

Gross Margin Rebounded, Yet Q4 Floor Points Down

Gross margin measures the share of sales left after the cost of processing nerve tissue and making surgical wraps. Breast reconstruction is Axogen's fastest-growing market, but those procedures use longer nerve grafts that cost more per millimeter to process than the company's pricing schedule recovers. Last quarter, that product mix pushed gross margin down to 72.7% and forced management to cut its full-year outlook.

In the third quarter, gross margin rebounded 2.1 percentage points to 74.8%, though it remained 1.8 percentage points below the 76.6% level of a year ago as cost of goods sold rose to 25.2% of revenue. Despite the rebound and a 74.2% gross margin across the first nine months, finance chief Lindsey Hartley kept the full-year outlook at a floor of at least 73%.

Holding that 73% floor while raising the revenue target to $290 million implies by our math that fourth-quarter gross margin could drop to 70.0%. Even if fourth-quarter margin stays higher, the unchanged floor contradicts the idea that third-quarter production efficiency solved the cost drag from longer breast grafts.

What to watch: fourth-quarter gross margin and any 2027 pricing or tissue-yield details on the fourth-quarter call. A fourth-quarter reading below 73% would show that fast breast growth still squeezes unit profitability heading into 2027.

๐Ÿ”ด๐Ÿ”ด strengthening CAPITAL ALLOCATION

Share Offerings and Stock Awards Diluted Per-Share Gains

On October 1, Axogen bought BioCircuit Technologies for $200 million in cash to add NerveTape, which the presentation describes as "a simpler, faster coaptation aid that eliminates the need for micro-suturing" when joining severed nerves. To pay for the deal before closing, the nerve-repair specialist sold 5.65 million new shares in a public offering for $225 million in net proceeds, its second stock sale of 2026.

  • Diluted share count: 61.5 million in the third quarter, up 25.3% from 49.1 million a year ago
  • Total common shares outstanding: 59.7 million at quarter-end, up 26.5% from 47.2 million at the end of 2025
  • Stock-based employee pay: $8.8 million, or 11.6% of third-quarter revenue, up from $5.4 million a year ago
  • General and administrative expense: rose 3.7 percentage points of revenue to 21.7%, including $2.0 million of BioCircuit deal costs
  • BioCircuit 2026 revenue contribution: at least $7 million in the fourth quarter against a $200 million purchase price

Because the share count grew by a quarter, net income excluding one-offs rose 113.6% from a year ago to $13.1 million, but profit per share excluding one-offs grew only 75.0% to $0.21. Meanwhile, stock-based pay consumed 65.7% of Ebitda excluding one-offs, keeping GAAP operating income down at $1.0 million.

What to watch: BioCircuit's 2027 revenue outlook and first-quarter 2027 diluted share count. If NerveTape sales do not grow fast enough to cover the 5.65 million new shares issued to buy it, per-share profit growth will continue to lag company-wide profit growth.

๐Ÿ”ด persistent DISCLOSURE

Risks this quarter didn't answer

Three standing concerns from prior quarters received no new dollar figures in the third-quarter earnings release or presentation slides.

  • Revenue by surgical market: the presentation states that breast grew over 50% and other markets grew steadily, but the quarterly filing or call remarks are needed to see exact dollar growth outside breast.
  • Dollar revenue from new insurance policies: coverage reached 96% of commercial lives, yet the release does not state how much third-quarter sales came from recently added Cigna and Elevance plans.
  • Manufacturing concentration: all Avance grafts are processed in Dayton, Ohio, and all Axoguard wraps come from one contract manufacturer, a setup that only a second supplier agreement would change.

Medicare Proposal Adds to 2026 Hospital Rate Gains

Medicare sets baseline payment rates for hospital outpatient departments and surgery centers. In January 2026, the agency raised outpatient facility payments for Avance nerve repairs by 40% in hospitals to $8,965 and by 35% in surgery centers to $6,157. In its third-quarter presentation, Axogen noted that Medicare's proposed 2027 payment rule increases core nerve repair rates again, while cutting payments for simple suture repairs by 30% to 46%.

Free cash flow (26Q3) $14.9 million
โ‡— accelerating

Free cash flow โ€” operating cash flow minus property and intangible purchases โ€” reached $18.9 million for the first nine months, up from negative $5.9 million a year ago. By our math, subtracting first-half free cash flow of $4.1 million leaves $14.9 million generated in the third quarter alone, or 19.6% of revenue, driven by higher operating profit and $13.4 million from higher payables.

Cash and investments (26Q3) $350.1 million
โ‡— accelerating

Cash, restricted cash, and investments rose by $236.7 million during the quarter from $113.4 million at the end of June. The medical-device group raised $225.0 million in net proceeds from its September stock offering and held zero long-term debt after paying off its credit facility in January. On October 1, management used $200.0 million of this cash to close the BioCircuit purchase, leaving $150.1 million by our math.

Accounts receivable and inventory (26Q3) $83.1 million combined
โ‡˜ decelerating

Customer bills awaiting collection ended September at $34.0 million, slightly below the $34.1 million recorded in June despite an 8.8% quarterly rise in revenue. Inventory stood at $49.1 million, up 3.7% from $47.3 million in June and up 15.8% from the end of 2025. Slower growth in working capital helped the surgical-implant maker turn more of its third-quarter profit into cash.

๐Ÿ”ฎ Guidance

FY26 Sales from businesses owned a year ago At least $283 million (at least +26% YoY)
๐Ÿ … raised from At least $279 million (at least +24% YoY)
โ‡— accelerating

Raised. The floor for sales excluding the new BioCircuit acquisition moved up by $4 million, or 2 percentage points of growth. By our math, that leaves at least $76.0 million for the fourth quarter, or 26.8% growth from a year ago. It is the third raise this year following increases of $4.3 million in the first quarter and $9.0 million in the second quarter.

FY26 Total Revenue (including BioCircuit) At least $290 million (at least +29% YoY)
๐Ÿ … raised from At least $279 million (at least +24% YoY)
โ‡— accelerating

Raised. Management added at least $7 million of fourth-quarter sales from BioCircuit, which closed on October 1, on top of the $4 million underlying raise. By our math, the $290 million floor implies at least $83.0 million of total fourth-quarter revenue, up 38.5% from $59.9 million in the fourth quarter of 2025.

FY26 Gross Margin At least 73%
๐Ÿ † unchanged from At least 73%
โ‡˜ decelerating

Unchanged. The firm kept the one-sided floor it set last quarter after cutting its original 74% to 76% target range. Because gross margin averaged 74.2% over the first nine months, the 73% floor leaves room by our math for fourth-quarter gross margin to fall as low as 70.0% as BioCircuit joins and longer breast grafts grow.

FY26 Free Cash Flow Positive
๐Ÿ † unchanged from Positive
โ‡’ stable

Unchanged. The nerve-repair group still expects positive free cash flow for the full year. With $18.9 million already banked through September and $14.9 million generated in the third quarter alone by our math, the annual target is already comfortably met barring a fourth-quarter cash outflow larger than $18.9 million.

โ“ Key Questions

What was third-quarter dollar revenue in each surgical market?

Management committed last quarter to reporting breast reconstruction separately, but the release and slides only state that breast grew over 50%. Investors need the exact dollar split to check whether the remaining two-thirds of the business in limb trauma and jaw surgery sped up or slowed.

What were BioCircuit's past twelve months of sales and margins?

Axogen paid $200 million in cash โ€” funded by issuing 5.65 million shares โ€” for a business expected to add at least $7 million in the fourth quarter. Knowing NerveTape's gross margin and operating cost is essential to see if the deal adds to or hurts 2027 margins.

Why does the gross margin outlook allow a fourth-quarter drop?

Third-quarter gross margin recovered to 74.8%, lifting the nine-month average to 74.2%. Keeping the full-year floor at 73% implies a fourth-quarter floor of 70.0% by our math, so management should explain how much of that gap is caution versus BioCircuit integration or graft costs.

How much will the 2027 prostate launch cost to run?

The company decided to start commercial development in prostate surgery in 2027 after completing 220 clinical procedures. Before the November 9 investor event, the open question is how many prostate specialists management will hire in 2027 and whether that spending will slow operating margin gains.

Where is the international expansion update promised for mid-2026?

On the third-quarter 2025 call, executives promised to update investors in the first half of 2026 on plans to enter overseas markets following US biologic approval. Three quarters after approval, the release still makes no mention of international regulatory filings or sales plans.
