Accuray (ARAY) Q1 2027 earnings review

Accuray cuts operating costs, but its machine orders fall half

Accuray, a maker of radiation therapy machines for cancer treatment, cut its operating costs while demand for its equipment fell. First-quarter sales dropped 8% as machine shipments slumped, though recurring service revenue rose 4% on higher contract prices. Chief executive Steve La Neve said fiscal 2027 is about commercial execution, and transformation cuts saved about $13 million, yet management still gave no formal full-year outlook.

Operating expenses, excluding one-offs$27.2M -29% from a year ago

⚖️ Verdict: 🔴 Bearish

The story got worse because new equipment orders fell in half to $20 million, even against an easy comparison from last year. By our math, $33 million of older orders aged out or cancelled, dragging the order backlog down 31% to $272 million. On the positive side, operating expenses excluding a goodwill write-down fell 29%, and stockholders approved a debt exchange that averts a financing penalty.

The question now is whether hospital demand for radiation machines can recover before the shrinking backlog pulls product revenue even lower. Either new partnerships with RaySearch and Samsung turn clinic interest into fresh orders, or ongoing age-outs swallow the cost savings and stall the service base. Second-quarter gross orders and backlog will show which way demand is moving.

🐂 Bull Case

🟢🟢 strengthening MARGIN

Cost Cuts Delivered $13 Million in Quarterly Gains

Accuray, which makes radiation therapy machines like CyberKnife and Radixact for treating cancer, launched a multi-phase restructuring last year to cut overhead and outsource work. Excluding a $35.4 million goodwill write-down, first-quarter operating expenses fell 29.0% from a year ago to $27.2 million, by our math.

The medical-device maker said its transformation programme generated about $13 million of cost and margin improvement in the quarter through "headcount reductions, more efficient usage of strategic partners, focused product development and marketing plans, and continued discretionary spending discipline". Every major expense line shrank as a share of sales.

  • Research and development: 9.0% of revenue, down 2.9 percentage points from a year ago by our math
  • Selling and marketing: 10.0% of revenue, down 2.4 percentage points by our math
  • General and administrative: 11.9% of revenue, down 1.5 percentage points by our math
  • Adjusted EBITDA (profit before interest, taxes, depreciation, and one-offs): $2.5 million, up from negative $4.1 million a year ago

These savings kept operating loss excluding the goodwill write-down to $1.1 million by our math, compared with $11.3 million a year earlier, even though revenue dropped 7.9%. What to watch: operating expenses in the second quarter. Staying near $27 million would show the leaner cost structure is permanent rather than a one-quarter pause in spending.

🟢 strengthening GROWTH

Service Pricing Lifted Recurring Revenue and Margins

While machine sales fluctuate with hospital capital budgets, Accuray collects recurring revenue from maintaining its installed systems. Service revenue rose 4.2% from a year ago to $59.2 million, or 5% in constant currency, and now makes up 68.4% of total sales by our math.

The company attributed the top-line gain to "service contract pricing actions and nearly 3% growth in the global installed base". Service gross margin — the profit left after paying field engineers and buying spare parts — rose 1.2 percentage points from a year ago to 35.1%, helped by "pricing actions and a lower service labor cost structure".

Higher service profitability lifted total gross margin by 1.4 percentage points to 30.1%. However, installed-base growth depends on shipping new machines, and product revenue fell 26.4% this quarter. What to watch: installed-base growth in the second quarter. A slowdown below 3% would mean weak machine deliveries are starting to cap the service business.

🐻 Bear Case

🔴🔴 persistent GROWTH contradicts narrative

Order Intake Cut in Half as Backlog Shrinks

"As I said on our last call, fiscal 2026 was a year of transformation, and fiscal 2027 is about execution," chief executive Steve La Neve said. La Neve added that the team is focused on "turning that engagement into orders and revenue, and on pairing the operating discipline we have built with stronger commercial performance".

First-quarter commercial numbers went the opposite way. Even against an easy comparison from a year ago when gross orders fell 28.5%, new gross product orders dropped another 49.8% to $19.8 million. Product revenue fell 26.4% to $27.3 million, and the book-to-bill ratio — gross orders divided by product revenue — sank to 0.7 from 1.1 a year earlier.

  • Gross product orders: $19.8 million, down from $39.6 million a year ago and $37.7 million last quarter
  • Orders removed via age-outs and cancellations: $33.1 million by our math, leaving net orders at negative $13.2 million
  • Order backlog: $272.0 million, down 31.3% from a year ago and down $40.6 million from last quarter by our math

Accuray removes orders from backlog if a hospital does not take delivery within 30 months. Because $33.1 million of older orders aged out or cancelled while only $19.8 million of new orders came in, the backlog shrank for an eighth straight quarter. What to watch: second-quarter gross orders. Anything below $40 million would keep backlog shrinking and depress future machine revenue.

🔴🔴 persistent CAPITAL ALLOCATION

Debt Recapitalization Passes, Locking In Heavy Dilution

Accuray has been reworking its balance sheet with its lender TCW to cut debt and avoid covenant defaults. Management said the deal is meant to "reduce leverage, enhance liquidity, and provide greater flexibility to execute our strategy". After the quarter ended, stockholders approved the proposals required to exchange $40 million of term loan debt for convertible preferred stock.

Modifying the loan terms triggered a $59.6 million non-cash accounting gain this quarter and lowered carrying debt on the balance sheet from $147.9 million to $101.7 million by our math. The approval removes the danger of a penalty fee and a covenant default, but it comes at a steep cost to existing common shareholders.

Diluted share count already rose 17.1% from a year ago to 139.3 million, and converting the preferred stock and warrants will roughly double the share base. Meanwhile, quarterly interest expense still jumped 46.9% from a year ago to $11.8 million. What to watch: interest expense and diluted share count in the second-quarter filing after the debt exchange closes. High remaining interest costs alongside a doubled share count would leave little profit for common shareholders.

🔴 new MARGIN

Goodwill Write-Down and Lower Machine Margins

For several quarters Accuray kept $57.9 million of goodwill — an accounting asset from past acquisitions — untouched on its balance sheet even as sales and orders declined. In the first quarter, the company recorded a $35.4 million non-cash goodwill impairment charge, cutting its remaining goodwill balance by more than half to $22.5 million.

At the same time, profitability on new radiation machines weakened. Product gross margin fell 1.5 percentage points from a year ago to 19.4% by our math, as fixed factory overhead spread across a 26.4% drop in product revenue. Excluding both the debt gain and the goodwill write-down, Accuray posted a net loss of $14.0 million.

What to watch: product gross margin in the second quarter. A reading below 20% would show that factory under-utilization and tariff costs are offsetting part of the company's operating expense cuts.

🔴 persistent DISCLOSURE

Risks this quarter didn't answer

Four standing operational and financial concerns received no new numbers in the first-quarter earnings release.

  • China tariffs and joint-venture deliveries: not quantified in the release; the quarterly filing's joint-venture note will show how many systems shipped and how much deferred margin remains.
  • Middle East shipment delays: cited as a risk factor with no dollar figure; the regional revenue table in the quarterly filing will show Europe, Middle East, and Africa performance.
  • Nasdaq minimum bid-price compliance: stockholders approved the reverse stock split after quarter end, and completion of the split before the February 2, 2027 deadline will settle the delisting risk.
  • Debt covenant holiday: runs through December 2027, so the first formal liquidity and leverage test will not arrive until March 2028.

RaySearch and Samsung Partnerships Advance Product Roadmap

Rather than building every software and imaging tool in-house, Accuray relies on outside partners to upgrade its radiation systems while keeping research spending down. In the first quarter, the company signed a definitive agreement with RaySearch to "accelerate its online adaptive radiation therapy strategy" and advanced its work with Samsung NeuroLogica on "volumetric imaging for the CyberKnife System", while eight-year prostate cancer study data backed CyberKnife outcomes.

Accounts receivable (27Q1) $50.3 million
⇘ decelerating

Uncollected customer bills fell 25.3% from last quarter's $67.4 million and 7.4% from $54.4 million a year ago, by our math. Receivables dropped faster than the 14.3% sequential decline in revenue, indicating that the company collected cash from fourth-quarter billings as machine shipments slowed.

Total deferred revenue (27Q1) $101.8 million
⇘ decelerating

Deferred revenue — cash customers have paid upfront for service contracts and installations not yet delivered — fell 8.6% from $111.3 million last quarter and 3.5% from $105.5 million a year ago, by our math. The drop shows Accuray is recognizing prior service prepayments faster than it is collecting new upfront cash.

Total cash and restricted cash (27Q1) $50.4 million
⇄ reversing

Cash and restricted cash rose by $1.6 million from $48.8 million at the end of June, after falling over the prior year from $69.9 million in the first quarter of fiscal 2026. The sequential increase relied on $15 million of financing proceeds from TCW in July rather than operating cash generation.

🔮 Guidance

FY27 Transformation Cost and Margin Improvement ~$13 million realized in Q1
🠆 unchanged from ~$15 million incremental FY27 improvement
⇗ accelerating

Unchanged. Accuray continues not to provide a formal revenue or Adjusted EBITDA outlook for fiscal 2027, focusing instead on its cost programme. The company generated about $13 million of year-over-year cost and margin gains in the first quarter, after realizing $20 million across all of fiscal 2026 against an initial $12 million plan.

❓ Key Questions

Why did gross orders fall to $20 million?

Gross product orders fell 50% from an already weak year-ago quarter to $19.8 million. Investors need to know how much of the drop came from China and the Middle East versus weaker hospital demand in the United States and Europe.

How much of the $272 million backlog is near 30 months?

By our math, $33.1 million of orders aged out or cancelled in the first quarter alone, leaving net orders at negative $13.2 million. Management has not disclosed how much of the remaining backlog is approaching the 30-month cutoff.

What is the exact share count after the TCW exchange?

Stockholders approved the $40 million debt-for-preferred exchange and reverse stock split after quarter end. Management should state the exact post-closing common and preferred share counts and whether the 8% preferred dividend will be paid in cash or stock.

When will management restore a full-year revenue and profit outlook?

Accuray withdrew its fiscal 2026 outlook in the third quarter of last year and still gives no formal revenue or Adjusted EBITDA targets for fiscal 2027. Restoring a numerical range would show that order and shipment timing has stabilized.
