Amphastar Pharmaceuticals (AMPH) Q3 2026 earnings review

Amphastar's top drugs hit records, but factory fixes squeeze profit

Amphastar's emergency nasal spray and asthma inhaler reached record sales, but factory quality fixes and older drug price cuts squeezed profit margins. The maker of complex injectable and inhaled medicines grew revenue 8% from a year ago to $208 million, and profit per share excluding one-offs rose 23% with help from a smaller share count. Chief executive Jack Zhang highlighted new launches, yet gross margin dropped to 48% as quality reviews at its IMS plant slowed epinephrine shipments.

โš–๏ธ Verdict: โšช Neutral

The story is unchanged because progress on BAQSIMI pricing was offset by deeper factory drag and a step back in the company's newest inhaler. The discount hit on BAQSIMI narrowed to $2.2 million from about $8 million in each of the prior two quarters, easing a major revenue worry. The bad news: gross margin missed the ~51% level framed on last quarter's call, and ipratropium bromide sales fell 21% from the second quarter in its first full quarter on the market.

The question now is whether new drug launches can offset both generic price erosion and IMS factory costs once initial exclusivity windows close. Either teriparatide and future pipeline approvals lift gross margin back above 50%, or generic competition on ipratropium bromide and glucagon keeps margins near 48%. Fourth-quarter gross margin and ipratropium bromide sales will settle it.

๐Ÿ‚ Bull Case

๐ŸŸข๐ŸŸข strengthening GROWTH

BAQSIMI Pricing Drag Narrowed

BAQSIMI, Amphastar's nasal spray for low blood sugar emergencies, drives the company's growth but has suffered from steep discount claims by hospitals and pharmacies. Earlier this year, those discount claims cut about $8 million per quarter from revenue and raised the risk that net prices would not recover.

In the third quarter, BAQSIMI sales rose 10% from a year ago to a record $58.8 million. Marketing efforts lifted unit volumes by $7.4 million. Lower selling prices still reduced sales by about $2.2 million, "due to achange in gross-to-net discounts due to changes in chargebacks and rebates and changes to the customer mix", the company said.

That $2.2 million price drag is less than a third of the $8.1 million hit in the second quarter. Narrowing the discount leak allowed unit growth to show up in revenue, even against last year's record third quarter.

What to watch: the fourth-quarter BAQSIMI price and volume breakdown in the annual filing. Another small price drag will confirm the discount fix is holding as the drug maker chases the $200 million annual sales level that triggers its next $100 million milestone payment to Eli Lilly.

๐ŸŸข strengthening CAPITAL ALLOCATION

Buybacks Cut Share Count 9%

Amphastar says the main goal of its share buyback program is to "offset dilution created by the Company's equity compensation programs". In practice, the drug maker is retiring far more stock than it issues to employees.

Diluted shares fell 9.0% from a year ago to 43.4 million, and on November 4 the board added another $50 million to the repurchase plan. Because the share count dropped so much, profit per share excluding one-offs rose 22.6% to $1.14, even though net income excluding one-offs grew only 10.2% to $49.3 million.

What to watch: fourth-quarter treasury stock purchases on the balance sheet. Continued heavy buying after this quarter's cash outflow will show whether executives prioritize shrinking the share count over rebuilding cash.

Price Hike Lifted Primatene MIST

Primatene MIST, the company's over-the-counter asthma inhaler, rebounded from an 8% second-quarter drop caused by retail stocking shifts. Third-quarter sales rose 9.2% from a year ago to a record $31.5 million because a second-quarter price increase added $1.6 million and store demand added $1.0 million.

Alongside its retail inhaler, the biopharmaceutical group expanded its generic development pipeline from two candidates in the first quarter to three, targeting a $1.2 billion market, while advancing four in-licensed proprietary compounds in research.

What to watch: fourth-quarter Primatene MIST sales against the company's full-year target of high single-digit growth. Through nine months, sales of the inhaler are up 1.9% by our math, so reaching the full-year goal requires a large fourth-quarter jump.

๐Ÿ”ด๐Ÿ”ด strengthening MARGIN contradicts narrative

Factory Fixes Cut Margin and Epinephrine Supply

"Our third quarter results demonstrate the strength of Amphastar's diversified portfolio and the commercial platform we have built," chief executive Jack Zhang said. On last quarter's call, finance chief Bill Peters said gross margin in coming quarters should stay near the second quarter's 50.8% because seasonal BAQSIMI sales would offset quality-fix costs at its International Medication Systems (IMS) subsidiary.

That offset did not hold. Gross margin fell to 48.0%, down 3.4 percentage points from a year ago and 2.8 points from last quarter. Cost of revenues rose 3.4 percentage points of sales to 52.0%, driven by lower prices on older drugs, California plant expansion costs, and "remediation efforts at our IMS facility in response to the FDA warning letter".

  • Gross margin: 48.0%, down from 50.8% in the second quarter and 51.4% a year ago
  • Epinephrine sales: $15.3 million, down 19% from a year ago
  • Reported profit margin: rose 11.3 points only because last year carried $28.4 million in legal and licensing charges; excluding those one-offs by our math, operating margin fell 3.6 points

The factory fixes also hurt revenue. Epinephrine injection sales dropped because of "slower product releases associated with ongoing remediation efforts at our subsidiary, International Medication Systems, Limited", and several other IMS drugs also fell "due to ongoing remediation activities".

What to watch: fourth-quarter gross margin and epinephrine sales. Another margin reading below 49% will show that IMS quality reviews and price cuts on older drugs are overwhelming the profit from high-season products.

New Inhaler Shrank in First Full Quarter

New generic launches are supposed to replace lost profit from older drugs and lift the company's gross margin. Teriparatide, an injectable bone drug launched in December 2025, did its part by reaching $6.4 million in third-quarter sales, up from $4.5 million in the second quarter.

However, ipratropium bromide, a generic breathing inhaler launched in April 2026, moved backward. Amphastar said the inhaler "continued to have strong sales during the quarter, following our launch in the second quarter of 2026". Yet its revenue dropped 21.0% from last quarter to $6.6 million, down from $8.4 million in the second quarter.

The drop matters because the third quarter was the inhaler's first full three months on the market and sat inside its initial 180-day exclusivity window, when generic competition is limited. Overall growth in the "other products" group also slowed to 9.5% from a year ago, down from 24.7% in the second quarter.

What to watch: fourth-quarter ipratropium bromide revenue as its six-month exclusivity window expires. A further drop will show that second-quarter launch stocking was the peak and that new generics are only replacing older drug declines rather than expanding total profit.

๐Ÿ”ด persistent CASH

Glucagon Erosion and Cash Outflow Persist

Rival generic competition continues to cut prices on Amphastar's older injectable glucagon emergency kit. Third-quarter glucagon revenue fell 36.8% from a year ago to $8.6 million, even though it compared against a year-ago quarter that had already fallen by nearly half.

At the same time, cash and short-term investments on the balance sheet dropped by $59.5 million during the quarter to $233.1 million, by our math. A $100 million milestone obligation to Eli Lilly fell due in the third quarter alongside ongoing share buybacks, leaving net debt at $379.9 million against $613.0 million of total debt.

What to watch: the third-quarter filing's cash flow statement and milestone liability footnote. Those tables will show how much of the $100 million Eli Lilly payout has already left the bank account and how much cash remains for factory upgrades.

๐Ÿ”ด persistent DISCLOSURE

Risks this quarter didn't answer

Three standing concerns got no new information in the third-quarter earnings release because the company gives its outlook and footnote details on the call and in its quarterly filing.

  • Full-year sales pace: no outlook appears in the release; hitting the prior 5% to 9% full-year growth target requires fourth-quarter sales to rise between 5.5% and 21.2% from a year ago by our math, after 4.8% growth through nine months.
  • Related-party drug licensing: the release does not update milestone or supply dealings with Hanxin and Letop, companies tied to the chief executive's family; the quarterly filing's related-party footnote will show any new payments.
  • 2027 insulin launch schedule: the release repeats that one biosimilar insulin candidate is filed with regulators but gives no update on approval timing or whether the IMS warning letter affects pending applications.

Rival Shortages Lifted Lidocaine and Ingredient Sales

Supply shortages at competing drug makers gave Amphastar a temporary volume boost in the third quarter. Lidocaine anesthetic sales rose 28.7% from a year ago to $16.6 million because rival suppliers could not meet hospital demand, reversing a 19% decline a year ago when competitors had stock. Active pharmaceutical ingredient sales from the company's Nanjing subsidiary in China also added $6.2 million to other product revenue.

Selling and administrative expenses (26Q3) $28.8 million
โ‡˜ decelerating

Overhead spending dropped from last quarter after surging in the first half. Selling and general administrative expenses totaled $28.8 million by our math, down from $31.6 million in the second quarter. Reported general administrative costs fell 57.3% from a year ago because last year included a $23.1 million legal settlement charge.

Accounts receivable (26Q3) $182.2 million
โ‡— accelerating

Uncollected customer bills grew much faster than revenue. Accounts receivable โ€” money owed by drug wholesalers and retailers for products already shipped โ€” rose 26.9% from $143.6 million at the end of December 2025, while third-quarter revenue was 13.4% higher than in the fourth quarter of 2025. Slower collection ties up working capital.

Operating cash flow (26Q3) $43.3 million
โ‡˜ decelerating

Cash generated by daily operations slowed from the second quarter's pace as customer receivables built up. Amphastar reported $142.5 million of operating cash flow for the first nine months, up 15.6% from a year ago. Subtracting the first half leaves $43.3 million for the third quarter by our math, down from $51.4 million in the second quarter.

โ“ Key Questions

Why did ipratropium bromide sales drop 21% in Q3?

The third quarter was the inhaler's first full quarter on the market after its April launch, yet revenue fell to $6.6 million from $8.4 million in the second quarter. Investors need to know how much was second-quarter stocking versus early price pressure.

How much did IMS factory delays cut Q3 sales?

Quality-review delays at the IMS facility drove a 19% drop in epinephrine sales and lowered gross margin to 48.0%. Management has not given the total dollar revenue delayed across all IMS products or when releases will normalize.

Does the full-year 5% to 9% sales plan stand?

Revenue is up 4.8% through nine months. Hitting the middle of the prior full-year plan requires 13.4% growth in the fourth quarter by our math, just as ipratropium bromide's six-month exclusivity window ends.

How is the Rancho Cucamonga plant expansion progressing?

Total net property and equipment barely moved since December ($312.9 million versus $310.6 million) after management redirected spending to IMS quality fixes. The quarterly filing's property footnote needs to show whether construction in progress rose above June's $34.8 million.
