ADMA Biologics (ADMA) Q3 2026 earnings review

ADMA Biologics' specialty drug grows fast, while its older medicine shrinks

ADMA Biologics, a maker of human plasma antibody drugs, is getting almost all its growth from one high-margin specialty treatment while its older drug shrinks. Reported sales rose 5%, but by our math underlying sales grew 17% after stripping out a one-time raw plasma sale from a year ago. Gross margin reached 71% on higher factory yields, and chief executive Adam Grossman raised the company's full-year profit floors.

At a glance
Sales excluding last year's one-time plasma sale+17% from a year ago by our math (reported revenue: +5% to $141.1M)
ASCENIV specialty drug sales$124.7M +31% from a year ago (88% of total sales)
Gross margin71% +14.5 percentage points from a year ago
Full-year profit outlook, excluding one-offsRaised $10M minimum net profit now at least $180M; revenue floor held at $530M

⚖️ Verdict: 🟢 Bullish

The story got better because specialty drug ASCENIV accelerated and now makes up nearly nine-tenths of total sales, shielding company profits from price wars in the standard antibody market. That shift allowed ADMA to lift its minimum earnings outlook without needing higher total revenue. One caution: sales of its standard drug BIVIGAM fell 21% from last quarter, and slower customer bill collections cut quarterly operating cash flow to $17 million by our math.

The question now is whether standard antibody price competition will stay confined to BIVIGAM or eventually slow ASCENIV's hospital adoption and cash collections. If ASCENIV stays insulated, its 31% growth and 71% margin will carry total revenue and cash flow higher; if distributors push back on its high price, unpaid bills will keep swelling. Fourth-quarter receivables and BIVIGAM sales will tell.

🐂 Bull Case

🟢🟢 strengthening GROWTH

ASCENIV Carries Underlying Growth to 17%

ADMA Biologics makes plasma-derived antibody treatments for people with weak immune systems. Its main product is ASCENIV, a specialty antibody infusion for complex immunodeficiency patients who still get sick on standard treatments. Chief executive Adam Grossman said growth came from "expanding physician adoption, new patient starts and growing end-user utilization".

  • ASCENIV sales: $124.7 million, up 30.7% from a year ago and 21.2% from last quarter
  • Total underlying sales: up 17.2% from a year ago by our math, stripping out last year's one-time $13.8 million raw plasma sale
  • ASCENIV revenue share: 88.4% of total company sales, up from 71.1% a year ago

Headline revenue grew only 5.1% because the pharmaceutical group did not repeat last year's spot plasma sale and its older drug shrank. Because ASCENIV now makes up nearly nine-tenths of total sales, its 30.7% growth rate will govern the top line once the older product stops falling. A new study of 127 complex patients also showed fewer hospital stays after starting the drug.

What to watch: fourth-quarter ASCENIV sales and any new full-year 2027 revenue target on the next earnings call. Sustaining sequential growth above 15% would confirm that new hospital accounts are adopting the drug faster than the standard market is slowing.

🟢🟢 strengthening MARGIN

Higher Factory Yield Lifted Gross Margin 14 Points

"Our third quarter results demonstrate ADMA’s ability to translate accelerating ASCENIV demand into substantial earnings growth," chief executive Adam Grossman said. Last year the company won regulatory approval for a manufacturing change that extracts about 20% more antibody medicine from each batch of human blood plasma.

That factory upgrade and the shift toward higher-priced ASCENIV cut the cost of making products by 14.5 percentage points of revenue, down to 29.2%. Gross margin rose from 56.3% a year ago to 70.8% this quarter. Even when compared against last year's 63.7% product gross margin—which strips out the money-losing spot plasma sale by our math—gross margin still expanded by 7.1 percentage points.

Operating profit margin rose 10.0 percentage points to 48.0%, even though research and development costs grew by 2.0 points of revenue and overhead rose by 2.6 points. The specialty biologics maker subsequently raised its full-year profit floors while leaving its revenue target unchanged.

What to watch: fourth-quarter gross margin against this quarter's 70.8% reading. A figure above 71% would show that high-margin ASCENIV mix is still lifting profitability beyond the initial factory yield step-up.

🟢 strengthening CAPITAL ALLOCATION

Buybacks Turn 31% Profit Growth Into 44% Per Share

ADMA set a target to buy back at least $200 million of its own stock during 2026. The company bought 2.7 million shares in the third quarter, bringing its nine-month total to 16.5 million shares, or 6.3% of its stock from the start of the year.

The drugmaker spent $180.3 million in cash on these buybacks over the first nine months, partly funded by drawing $125 million from its bank credit line. That reduced the basic share count by 6.2% from a year ago and the diluted count by 7.7%. Because fewer shares split the profit, basic earnings per share excluding one-offs rose 43.8% to $0.23, outpacing the 31.2% rise in net profit excluding one-offs.

What to watch: fourth-quarter share repurchases and revolving debt repayments in the annual filing. Completing the remaining $20 million of buybacks without drawing more debt would prove the company can fund capital returns entirely from quarterly cash flow.

🐻 Bear Case

🔴🔴 persistent COMPETITION contradicts narrative

BIVIGAM Sales Fell Again Despite Stabilization Claims

"We are also encouraged by signs of stabilization in underlying BIVIGAM utilization, despite competitive pressures in the standard immune globulin market," chief executive Adam Grossman said. BIVIGAM is the company's standard antibody infusion, which competes directly against larger plasma fractionators in a well-supplied U.S. market.

The quarter's numbers cut against that stabilization framing. BIVIGAM revenue dropped 35.2% from a year ago and fell 20.8% from the second quarter to $15.4 million. That sequential drop erased the second quarter's recovery to $19.4 million and put sales right back at the first quarter's $15.4 million trough. The release blamed "continued competitive pricing and channel inventory pressures".

Management said it is choosing "disciplined commercial participation rather than matching aggressive competitor rebating and discounting". Walking away from low-price volume protects company gross margin, but it leaves ADMA reliant on one drug for all of its growth while rivals discount heavily to clear excess inventory.

What to watch: fourth-quarter BIVIGAM revenue against the $15.4 million trough level. Another drop below $15 million would show that hospital and clinic demand is still shrinking rather than stabilizing.

🔴 persistent CASH

Unpaid Customer Bills and Inventory Absorb Cash

ASCENIV costs roughly six times as much per infusion as standard antibody drugs, which strains the working capital of the distributors and clinics that buy it. Earlier this year, finance chief Terry Kohler targeted collecting customer bills within 90 to 105 days during the second half of 2026.

Instead, unpaid customer bills rose to $169.2 million in the third quarter, while inventories climbed to $266.1 million, up 35.3% from a year ago. By our math, accounts receivable equaled 108 days of sales, up from about 101 days in the second quarter and above the company's target range. Operating cash flow slowed to $17.4 million in the third quarter by our math, down from $30.0 million in the second quarter and $58.0 million in the first.

What to watch: fourth-quarter accounts receivable and days sales outstanding in the annual report. A drop back below 100 days would confirm that the McKesson Specialty distribution partnership is speeding up customer collections as promised.

🔴 persistent DISCLOSURE

Risks this quarter didn't answer

Three standing operational questions received no new numbers in the third-quarter earnings release.

  • Distributor inventory levels: management relies on distributor-reported patient pull-through data to judge channel inventory, and the release gives no days-on-hand figure.
  • McKesson Specialty ramp: the company gave no revenue or order figures showing how fast its new distributor is adding oncology and transplant clinics.
  • High-titer plasma collections: fewer than 10% of plasma donors carry the antibodies needed for ASCENIV, and the release did not quantify third-party collection volumes.

FDA Feedback Moves Pneumonia Drug Toward Trials

ADMA is developing SG-001, an antibody treatment to prevent severe pneumonia in patients with weak immune systems, which it estimates could bring in $300 million to $500 million a year if approved. Chief executive Adam Grossman said the company received "constructive feedback from the FDA regarding the proposed clinical development strategy" and confirmed its laboratory testing method is acceptable, keeping research spending elevated at $4.5 million this quarter.

💲 Other KPIs

Selling, general and administrative expense (26Q3) $26.6 million
⇒ stable

Overhead and sales spending held steady from last quarter's $26.7 million, but rose 22.0% from a year ago. As a share of revenue, it increased 2.6 percentage points from last year to 18.8%. The company said the increase came from "additional personnel and professional fees, as well as legal fees associated with ongoing litigation related matters".

Net debt (26Q3) $71.3 million
⇘ decelerating

Total debt stood at $195.9 million against $124.6 million in cash, leaving net debt at $71.3 million by our math. Total debt rose from $72.1 million at the end of 2025 because the biologics group borrowed $125.0 million from its JPMorgan credit line in the first half to fund share buybacks, driving quarterly interest expense up to $3.4 million.

🔮 Guidance

FY2026 Total Revenue At least $530 million
🠆 unchanged from $530–560 million
⇗ accelerating

Unchanged at the floor. The plan calls for at least $530 million in sales, or at least 3.9% growth for the year by our math. After $380.0 million in the first nine months, meeting the $530 million floor requires at least $150.0 million in the fourth quarter, which equals 7.8% growth from a year ago and 6.3% sequential growth.

FY2026 Adjusted EBITDA, excluding one-offs At least $275 million
🠅 raised from $265–300 million
⇘ decelerating

Raised at the floor. ADMA lifted the bottom of its cash operating profit target by $10 million, pointing to at least 19.0% full-year growth by our math. Because the drugmaker already earned $198.8 million in the first nine months, the $275 million floor leaves just $76.2 million for the fourth quarter—up 3.6% from a year ago and down slightly from this quarter's $77.3 million.

FY2026 Adjusted Net Income, excluding one-offs At least $180 million
🠅 raised from $170–200 million
⇘ decelerating

Raised at the floor. The group lifted its minimum net profit target excluding one-offs by $10 million, to at least 11.9% full-year growth by our math. With $130.7 million banked through September, the $180 million minimum leaves $49.3 million for the fourth quarter by our math, down 6.3% from last year's fourth quarter and below this quarter's $51.1 million.

❓ Key Questions

Why did BIVIGAM revenue drop 21% sequentially if demand stabilized?

BIVIGAM sales fell from $19.4 million in the second quarter back to $15.4 million in the third, matching the first-quarter trough. Investors need to know how much of that drop came from price cuts versus distributors reducing their warehouse inventory.

Why did days sales outstanding rise back above the target?

Unpaid customer bills climbed to $169.2 million, pushing collection times to roughly 108 days by our math—above the 90 to 105 day second-half target set last quarter. Management should explain whether distributor payment concessions are still in place.

When will ADMA restore full-year 2027 financial targets?

The company withdrew its long-term targets in the first quarter due to standard antibody price competition. With ASCENIV now making up 88.4% of revenue and margins above 70%, management should clarify what visibility it needs before issuing a 2027 outlook.

What are the expected trial costs and timelines for SG-001?

Now that the FDA has given feedback on the clinical study strategy and testing assay for SG-001, investors need to know when the formal trial application will be filed and how much research spending will rise in 2027.
